Clinical Pharmacist
About The Position
Clinical pharmacist works daily and in coordination with Pharmacy Clinical Coordinator to support evidence based medication use throughout the health system. The Clinical Pharmacist will provide pharmaceutical care, including medication therapy consultation, monitoring, patient specific regimen optimization, adverse event investigation and drug side effect mitigation. Additional program support duties for the organization will include antimicrobial stewardship, drug education and training to Medical, Nursing and Pharmacy staff, and other projects and programs as designated by the pharmacy management team.
Requirements
- PharmD OR Bachelor of Pharmacy Degree
- Current CA Pharmacist license
- Pharmacy degree or equivalency exam
- Completion of practice-relevant Board of Pharmacy Specialties (BPS) certification, e.g., Board Certified Pharmacotherapy Specialist (BCPS)
- Current Advanced Cardiac Life Support (ACLS) Certification

- Four (4) years acute care hospital pharmacy experience
