About The Position

Hartford Hospital is hiring a Clinical Pharmacist for its Inpatient Pharmacy Department. This role focuses on ensuring optimal drug therapy for patients in the clinical specialty area of thrombosis and hemostasis, and providing educational activities to pharmacy staff and other caregivers to ensure safe and cost-effective drug therapy. Requirements

  • Current CT Pharmacist license required.
  • PharmD required.
  • Completion of an ASHP approved pharmacy residency program or Minimum 3 to 5 years pharmacy experience required.

Nice To Haves

  • Board certification in pharmacy is preferred.
  • Two years of hospital pharmacist experience preferred.

Responsibilities

  • Serves as the health system subject matter expert for thrombosis, anticoagulation, hemostasis, and blood factor therapies.
  • Provides strategic oversight for policies, procedures, protocols, and clinical guidelines related to anticoagulants, antiplatelet agents, reversal agents, coagulation factor products, and massive hemorrhage management.
  • Leads the development, implementation, and maintenance of evidence-based practices for anticoagulation management and reversal, peri-procedural antithrombotic therapy, bleeding emergencies, factor replacement therapy, and management of inherited and acquired bleeding disorders.
  • Oversees institutional standards for anticoagulation therapies including warfarin, unfractionated heparin, enoxaparin, direct oral anticoagulants (DOACs), and other parenteral anticoagulants.
  • Provides operational and clinical oversight for blood factor and hemostatic therapies, including 4-factor PCC, coagulation factor concentrates, recombinant clotting factors, fibrinogen replacement products, von Willebrand factor products, and other specialized hemostatic and reversal agents.
  • Serves as the primary pharmacy consultant for complex bleeding events, anticoagulation reversal, coagulation disorders, and blood factor utilization across the inpatient setting.
  • Guides appropriate utilization of high-cost and high-risk therapies to optimize patient outcomes, patient safety, regulatory compliance, and stewardship of specialty medications.
  • Collaborates with Hematology, Transfusion Medicine, Trauma, Critical Care, Surgery, Cardiology, Neurology, Emergency Medicine, and Perioperative Services to standardize thrombosis and hemostasis practices throughout the organization.
  • Provides expertise regarding antithrombotic therapies, including warfarin, heparin, enoxaparin, DOACs, cangrelor, ticagrelor, and other anticoagulant and antiplatelet agents.
  • Supports institutional emergency response pathways, anticoagulation stewardship programs, and massive transfusion protocols through recommendations on factor replacement, reversal strategies, and hemostatic therapies.
  • Oversees formulary management, medication-use evaluations, quality improvement initiatives, and clinical outcomes related to thrombosis, anticoagulation, blood factors, and hemostatic agents.
  • Leads education and training for pharmacists, physicians, nurses, residents, and students on thrombosis, hemostasis, anticoagulation, blood factor therapies, and emerging treatment options.
  • Evaluates new evidence, medications, and technologies to advance thrombosis and hemostasis care and support system-wide clinical excellence.
  • Other duties as assigned.
