About The Position

Corewell Health Grand Rapids Hospitals Retail Pharmacy is a closed-door, outpatient pharmacy located at Butterworth Hospital in Grand Rapids, Michigan. This pharmacy operates under a “Specialty-Plus” Model, dispensing both Specialty and Mon-Specialty Medications pertinent to specific disease states that often require a higher level of care. This frequently involves a multidisciplinary, collaborative approach to ensure proper medication coordination and administration. Patient populations currently served include Hemophilia and Cystic Fibrosis. This is a wonderful opportunity to join an engaging, growing pharmacy space as we expand to provide superior, comprehensive care to a larger patient population.

Requirements

  • Bachelor's Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy
  • 2 years of relevant experience hospital pharmacy
  • State of Michigan Pharmacist license (required within 90 days)
  • State of Michigan Pharmacist Controlled Substance license (required within 90 days)
  • Basic Life Support (BLS) certification from American Heart Association (required within 180 days)

  • American Society of Health-System Pharmacists (ASHP) accredited residency in Pharmacy Practice
  • Adv Cardiovascular Life Support (ACLS) certification from American Heart Association (required within 180 days)
  • Pediatric Adv Life Support (PALS) certification from American Heart Association (required within 180 days)
  • PharmD Degree and/or PGY1 Residency and/or PGY2 in Retail / Community / Ambulatory Pharmacy
  • Strong understanding of retail or outpatient pharmacy operations
  • Ability to proactively ensure compliance with requirements
  • Ability to critically evaluate internal processes for continuous improvement
  • Flexibility and ability to adapt to a changing environment

Responsibilities

  • Provides pharmaceutical care according to professional and departmental standards and statutory requirements.
  • Provides safe, effective, rational, and cost-conscious pharmacotherapeutic recommendations through the department's medication management programs.
  • Assists in supervising the functional activities of Pharmacy Technicians and Interns.
  • Has state of Michigan pharmacist in charge responsibilities for this pharmacy location including compliance to all regulatory requirements.
  • Maintains accountability for ensuring the safety and efficacy of medication use.
  • Educates and acts as a consultant to patients/family and members of the health care team related to optimal drug therapy and participates in process improvement activities and drug use evaluations.
  • Works with multiple medication automation systems.
  • Interprets and screens medication orders for appropriate agent, dose, route, allergies, and potential drug interactions.
  • Safely compounds and dispenses medications for administration to patients.
  • Evaluates patients, orders appropriate laboratory tests, and interprets serum drug levels utilizing pharmacokinetic principles; provides prescribers with dosage regimen recommendations to optimize therapeutic outcomes, reduce patient risk, and monitor the costs associated with drug therapy.
  • Provides appropriate supervision and direction within appropriate skill level to technicians, residents, interns, and other support personnel.
  • Engages in continuous study of the entire professional field to maintain the professional competence, knowledge, and skills necessary for satisfactory performance of all assigned responsibilities.
  • Completes required continuous training and education, including department and Spectrum Health specific requirements on a timely basis.
  • Provides education and counseling to patients/family/significant others/members of the health care team in order to optimize outcomes and appropriate use of medications.
  • Ensures that services are provided in accordance with State and Federal regulations, organizational policy, and accreditation/compliance requirements.
