About The Position

Vanderbilt University Medical Center (VUMC) is seeking a Clinical Pharmacist for its Specialty Pharmacy team. This role provides leadership within a specialized healthcare team to ensure optimal and cost-effective drug therapy for patients. The Clinical Pharmacist will develop pharmacotherapeutic goals, recommend regimens, and create monitoring plans tailored to individual patient needs. A key responsibility includes leading efforts in preventing and reporting medication errors and adverse drug reactions, contributing to continuous process and practice improvement to enhance medication therapy safety. This position will primarily support Gastroenterology and Liver clinical spaces (IBD, Gastroenterology, Hepatology, Infectious Disease) but will also float to various disease states. Daily tasks involve ensuring patient medication access through benefit investigations, prior authorization/appeal completion, enrollment in patient assistance programs, and addressing barriers. Additionally, the role focuses on ensuring patient compliance and efficacy through timely assessments and chart reviews.

Responsibilities

Reviews drug orders and/or prescriptions to ensure the safe, appropriate, and cost-effective use of medications.
Prepares and dispenses medications, provides drug information, and manages medication therapy in accordance with federal, state, and local regulations and hospital policy.
Provides outstanding patient care by optimizing medication preparation and resolving medication-related problems.
Maintains the quality, safety, and consistency of clinical pharmacy services in assigned areas.
Supervises the activities of assigned support personnel and verifies the accuracy and appropriateness of all medications prepared by support personnel prior to dispensing to patients or dispensing to healthcare providers for administration to patients.
Ensures patients obtain medication access via completion of Benefit Investigation, PA/Appeal Completion, Enrollment in Patient Assistance Programs, and addressing any barriers reported.
Ensures compliance and efficacy by completing timely patient assessments and chart review.
