About The Position

Provides direct operational and clinical patient services and collaborates with a multidisciplinary team to improve care, achieve optimal outcomes, and provide continuity of care. The role delivers pharmacy patient-care services, including therapy care-plan development, medication management, education, monitoring, and follow-up, with evidence-based and cost-effective comprehensive medication management for patients with chronic conditions. To support collaborative care, pharmacists may obtain patient medical histories, order and evaluate laboratory results, and make medication decisions—including modifying or discontinuing treatment—within their scope of practice. The role also supports departmental operations and clinical programs, participates in committees and education, promotes the pharmacy profession, and mentors pharmacy students and/or residents. Operational and clinical service allocation depends on patient needs, staffing, and pharmacy workload. The position requires weekends, required evenings, and holidays.
