About The Position

This is a temporary, benefitted opportunity for a pharmacist who wants to take a more focused, patient-centered approach to care. In this role, you’ll assume primary responsibility for medication management for a dedicated group of patients. You’ll work closely with clinical teams to ensure therapies are safe, effective, and tailored to each individual—making a direct and meaningful impact every day. Requirements

  • PharmD (or equivalent) and current pharmacist licensure (or eligibility)
  • Experience in a clinical or hospital pharmacy setting
  • Strong clinical judgment and attention to detail
  • Excellent communication and collaboration skills
  • A patient-first mindset with a commitment to quality care
  • Ability to manage responsibilities independently while working as part of a team
  • License to practice as a Registered Pharmacist in the State of New York
  • Doctor of Pharmacy degree; or Master’s degree and the equivalent of one (1) year of full-time clinical pharmacy or pharmacy-related experience; or Bachelor’s degree and the equivalent of three (3) years of full-time clinical pharmacy or pharmacy-related experience.

  • Completed PGY1 Pharmacy Practice Residency
  • Board certified
  • In the process of completion of PGY2 Pharmacy Specialty Residency Program
  • 3+ years of work experience in a position of Clinical Pharmacist Specialist (PGY1 Pharmacy Residency accounted for 3 years of clinical experience)
  • Contributions to scholar activities (within the last 3 years) such as research, publications, and poster presentation.
  • Experience in developing programs contributing to pharmacy practice and patient safety and experience.

Responsibilities

  • Serve as a key partner in patient care, helping guide medication decisions and ensure the best possible outcomes.
  • Take ownership of medication management for a core group of patients.
  • Review and monitor medication therapies to ensure safety and effectiveness.
  • Collaborate with physicians, nurses, and care teams on treatment plans.
  • Provide recommendations to optimize therapy, including cost-effective options when appropriate.
  • Counsel patients on how to use their medications safely and confidently.
  • Document and track medication-related interventions and outcomes.
