About The Position

The Clinical Pharmacist Specialist provides clinical pharmacy services professionally and competently to assure rational and appropriate drug therapy throughout the Health System. This role involves a broad scope and depth of pharmacotherapy knowledge and clinical skills, independent decision-making for complex drug therapy selection, and collaboration with healthcare providers across the Health System to provide patient care and medication consultation services. The specialist will develop, recommend, and monitor therapeutic plans, participate in patient care rounds, and contribute to multidisciplinary teams in developing protocols, order sets, and critical pathways. They will also lead drug quality programs, drug usage evaluations, and research, presenting findings at professional meetings. Responsibilities include responding to complex drug information requests, evaluating and summarizing literature for formulary reviews and patient care decisions, implementing policies and procedures for regulatory compliance, and detecting, preventing, monitoring, documenting, and reporting adverse drug events and medication errors. The role also requires responding to patient care urgencies/emergencies and effective communication.

Requirements

  • Completion of residency OR Fellowship training OR Board certification Through the Board of Pharmaceutical Specialties.
  • The employee may be working towards board certification as part of a timeline mutually agreed upon by the employee and supervisor.
  • Licensure as a Pharmacist in New York State is required.
  • BS: Pharmacy (Required)
  • LPharm - Licensed Pharmacist - New York State Education Department (NYSED)
  • M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

Responsibilities

  • Independently responsible for complex decision-making, such as drug therapy selection
  • Works in collaboration with others across the Health System providing patient care and medication consultation services.
  • Able to develop, recommend, and monitor therapeutic plans.
  • Function with other Health Care Providers on bed-to-bed patient care rounds.
  • Function on Health System multidisciplinary teams in the development of protocols, order sets, and critical pathways to optimize patient care.
  • Develop, conduct, and participate in drug quality programs, drug usage evaluations, and research.
  • Present findings at appropriate local or national professional meetings.
  • Able to respond to complex drug information requests using literature searching expertise.
  • Evaluate and summarize primary literature and incorporate the information into formulary reviews and patient care decisions.
  • Works with pharmacy management and specialty departments to implement policies and procedures to ensure compliance with all applicable regulations (for example: state and federal law, accrediting bodies and National Patient Safety Goals).
  • Detect, prevent, monitor, document, and report adverse drug events and medication errors.
  • Respond to patient care urgencies/emergencies within your area of expertise.
  • Communicate effectively, both orally and in writing.
  • Take a leadership role in teaching prescribers, pharmacists, and/or nursing staff throughout the Health Care System through formal teaching programs and informal meetings.
  • Precept Pharmacy Interns and Residents as appropriate.
  • Committed to continual learning of new knowledge and the enhancement of critical thinking and problem-solving skills.
  • Attend at least 1 national professional meeting annually.
  • Performs other duties as assigned.
