This role is with UChicago Medicine, a world-class academic healthcare system. The Clinical Pharmacist Specialist in Pediatric Hematology-Oncology will be responsible for providing safe, effective, and prompt medication therapy. This includes direct patient care for pediatric hematology-oncology inpatient and ambulatory care services. The individual will deliver optimal medication therapy to pediatric patients with a broad range of disease states in hematology, oncology, and hematopoietic stem cell transplant (HSCT). They will also support decentralized medication use systems. Clinical Pharmacy Specialists provide direct patient-centered care and integrated pharmacy services in a decentralized practice setting, collaborating with physicians, nurses, and other hospital personnel. This involves prospective chart review, education for providers and patients, multidisciplinary team communication, and participation in patient care activities. Specialists are aligned with interdisciplinary programs to manage medication therapy and ensure pharmaceutical care is integrated throughout the institution. They participate in all aspects of the medication-use system, providing comprehensive and individualized pharmaceutical care. Services include prospective patient review, needs assessment, age and disease-specific drug therapy and patient education, adjustment of care based on patient response, and clinical interventions to prevent adverse events. They serve as departmental resources and liaisons, manage medication use according to standards, and conduct clinical research, practice advancement projects, and quality improvement initiatives. They also provide education and training and precept pharmacy students and residents.
