About The Position

Join UW Health's team of respected experts in medication therapy management as a Clinical Pharmacist – Pediatrics. This role involves delivering comprehensive pediatric medication therapy management within an interdisciplinary inpatient care team, ensuring drug therapy is safe, effective, efficient, and cost-effective. The pharmacist will actively participate in bedside medical emergencies, providing expertise in medication selection, preparation, procurement, documentation, and medication history review. Key responsibilities also include teaching and precepting Pharmacy Residents and PharmD students, engaging in interdisciplinary patient care rounds, and contributing to the advancement of pharmacy practice through guideline development and quality improvement activities. The position is based at American Family Children’s Hospital in Madison, Wisconsin, a Magnet®-designated facility offering specialized pediatric care, including a Level One Pediatric Trauma Center and Level IV NICU.

Requirements

  • Bachelor's Degree in Pharmacy or a PharmD degree
  • 1 year of clinical patient care and orders management experience
  • Licensed to practice pharmacy in the State of Wisconsin within 90 Days
  • Basic Life Support (BLS) within 90 Days
  • Pediatric Advanced Life Support (PALS) within 180 Days

Nice To Haves

  • Completion of an ASHP Accredited Residency training program
  • 3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program
  • Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.)
  • Inpatient pediatric experience and board certification or pediatric residency training

Responsibilities

  • Deliver comprehensive pediatric medication therapy management as part of an interdisciplinary inpatient care team to ensure drug therapy is safe, effective, efficient, and cost effective.
  • Actively participate in bedside medical emergencies, providing expertise in medication selection, preparation, procurement, documentation, and medication history review.
  • Teach and precept Pharmacy Residents and PharmD students through structured practical experiences and formal didactic education.
  • Engage in interdisciplinary patient care rounds, including medication reconciliation, therapy monitoring, order verification, patient and caregiver education, drug information support, and individualized drug dosing.
  • Contribute to the advancement of pharmacy practice through the development and implementation of clinical practice guidelines, order sets, and standardized practice protocols, as well as participation in scholarly and quality improvement activities.
