The Oncology/Infusion Clinical Pharmacist provides advanced clinical pharmacy services for patients receiving cancer therapy and/or specialty infusion therapies across inpatient and outpatient settings. In collaboration with the multidisciplinary healthcare team, this role ensures safe, accurate, and evidence-based chemotherapy and supportive care by verifying and interpreting medication orders, managing complex regimens, documenting clinical interventions, and proactively identifying and resolving medication-related issues. Key responsibilities include developing and implementing clinical protocols, managing complex chemotherapy regimens, and contributing to infusion/oncology-focused medication safety initiatives by ensuring proper handling, dosing, and administration of hazardous drugs. In addition, the Oncology/Infusion Clinical Pharmacist actively monitors therapy outcomes, screens for drug interactions and contraindications, provides timely recommendations to prescribers to enhance patient safety and therapeutic efficacy, supports transitions of care by reconciling oncology-related medications, provide drug information and patient education, and participating in quality improvement efforts that enhance clinical outcomes and workflow efficiency. In outpatient infusion services, the pharmacist supports patient access and revenue cycle processes – including prior authorization, reimbursement coordination, and patient assistance resources – while providing oversight of oncology compounding operations to ensure safe preparation practices and regulatory compliance. The Oncology/Infusion Clinical Pharmacist also contributes to staff training and the effective use of technology to strengthen chemotherapy safety across the continuum of care.
