About The Position

This role is more than just a job; it's an opportunity to provide patient-focused services, specializing in pharmaceutical care for patients receiving chemotherapy. The Clinical Pharmacist will evaluate chemotherapy regimens for appropriateness, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions. They will communicate with prescribers regarding medication order changes, document these changes, and inform relevant parties. Patient consultations for chemotherapy and drug information will be provided as requested. The role involves detecting, monitoring, documenting, and reporting adverse drug reactions and medication errors. Additionally, the pharmacist will develop, monitor, and maintain oncology services, collaborating with physicians, pharmacy leadership, nursing, clinical laboratory staff, and other healthcare professionals. Key responsibilities include developing interventional strategies to optimize chemotherapy utilization, improving patient outcomes through the creation of oncology guidelines, clinical pathways, restriction criteria, policy development, new laboratory methods, and formulary management. The pharmacist will establish systems for reporting and monitoring chemotherapy utilization and effectiveness metrics, identify opportunities for improvement in chemotherapy use, lab monitoring, and communication, and conduct medication use evaluations and prospective audits. Active participation in hematology/oncology committees, pharmacy and therapeutics committees, and other appointed committees is expected. The role also includes performing formulary evaluations and chemotherapy class reviews for anti-neoplastic agents, presenting findings to the P&T committee, and documenting all activities and interventions for cost savings and outcome purposes. Collaboration and ongoing education of hospital and pharmacy department staff are crucial, fostering teamwork and ensuring high-quality care delivery. This includes developing educational materials, competencies, and providing in-services. The pharmacist must demonstrate awareness of medical record documentation, TJC standards, State Board of Pharmacy requirements, departmental and hospital policies, and health-system initiatives.

Requirements

  • 2 to 3 years of experience in inpatient pharmacy hospital/clinical capacity
  • Currently licensed or registered to practice pharmacy in Louisiana.
  • Pharm.D.
  • PGY 1 Residency Program

Nice To Haves

  • Board Certification in the applicable specialty area preferred.
  • PGY 2 in Oncology, preferred

Responsibilities

  • Participates in the provision of patient-focused services.
  • Provides pharmaceutical care to patients receiving chemotherapy.

  • Discusses medication order changes/clarifications with the prescribers, documents any changes in patient and pharmacy records, and informs others of medication order changes.
  • Provides patient consultations as requested for chemotherapy and drug information.
  • Detects, monitors, documents, and reports adverse drug reactions and medication errors.
  • Develop, monitor, and maintain oncology services.
  • Collaborates with hematology/oncology physicians, pharmacy clinical directors/managers, nursing, clinical laboratory staff, and other healthcare professionals.
  • Develop interventional strategies to optimize appropriate chemotherapy utilization.
  • Improve patient outcomes by creating oncology guidelines, clinical pathways, restriction criteria, policy development, new laboratory methods, and formulary management.
  • Establishes a means to report and monitor ongoing chemotherapy utilization and identified metrics showing the effectiveness of oncology services.
  • Identify opportunities for improvement regarding chemotherapy use, lab monitoring, and communication, including but not limited to performing medication use evaluations (MUEs or QA) and conducting prospective audits.
  • Actively participates on the hematology/oncology committees, pharmacy and therapeutics committees, and other appointed committees as needed.
  • Performs formulary evaluation and chemotherapy class reviews for anti-neoplastic agents and presents to the P&T committee.
  • Documentation of all activities/ interventions for cost savings and outcome purposes.
  • Collaboration and ongoing education of hospital and pharmacy department staff.
  • Collaborates with hospital and pharmacy staff to insure timely resolution of problems and questions.
  • Fosters teamwork and takes responsibility to ensure delivery of high-quality care.
  • Provides ongoing education to hospital staff and pharmacists by developing educational material and competencies and providing in-services as needed.
  • Demonstrates awareness and understanding of documenting information in the medical record, TJC standards, and State Board of Pharmacy requirements.
  • Demonstrates awareness and understanding of departmental and hospital policies, and health-system initiatives.
  • Directs and initiates development, implementation and maintenance of clinical pharmacy services.
  • Provides clinical consultation to the Medical Staff and other health care providers within the appropriate team or unit(s), suggests appropriate cost-effective therapeutic alternatives as needed.
  • Communicates and documents all activities/interventions (i.e., medication order changes/clarifications with the prescriber, pharmacokinetic dosing/monitoring, adverse drug reactions and errors etc.).
  • Fosters teamwork and takes responsibility to ensure delivery of high-quality patient care.
  • Performs staff duties as needed.
  • Collaborates with all health care providers to ensure timely resolution of concerns and questions.
  • Provides ongoing education and timely drug information to physicians, nursing, pharmacists, and other healthcare professionals by developing educational materials and competencies and providing in-services as needed.
  • Facilitates the development and implementation of treatment guidelines, pathways, protocols, formulary changes, medication use evaluations, and any other appropriate drug information reports as approved by the appropriate committee(s).
  • Actively participates on clinical committees including pharmacy and therapeutics committee and other appointed committees as needed.
  • Acts as a liaison to other healthcare practitioners.
  • Fosters a good working relationship with physicians, medical staff, nursing, and other healthcare professionals.
  • Demonstrates awareness and understanding of documenting information in the medical record, JCAHO and CMS standards, and State Board of Pharmacy requirements.
  • Ensures compliance with ACHC and URAC Specialty Pharmacy Accreditation Standards and participates in audit, quality improvement projects and ongoing accreditation readiness activities.
  • Lead and support completion of prior authorizations and appeals.
  • Collaborate with providers, payers and patient access teams and maintain current knowledge of payer policies and formulary restrictions.
  • Ensure prescriptions align with 340B patient definition, eligible providers and covered entity sites; partners with 340b operations team and document clinical workflows that directly impact 340B compliance outcomes.
