About The Position

The Clinical Pharmacist Internal Medicine will oversee pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens. They will use effective patient education techniques to provide counseling to patients and caregivers on medication therapy, adverse effects, compliance, appropriate use, handling, and medication administration. The role involves designing, recommending, monitoring, and evaluating patient-specific pharmacotherapeutic regimens based on relevant biomedical literature, implementing recommendations effectively, and utilizing policies and procedures. This position also includes reviewing and interpreting medication orders and prescriptions, participating in the management of medical emergencies, and actively supporting department medication safety and quality improvement activities. The pharmacist will perform quality assurance and performance improvement of the medication use system, collaborate with the healthcare team to manage formulary adherence and the medication-use process, and assist in the acquisition of non-formulary, compassionate use, and investigational medications. Practice must be in compliance with organization’s policies and procedures, as well as local, state, and federal regulations. The role also involves preparing, dispensing, and overseeing the proper storage and handling of medications. Additionally, the Clinical Pharmacist will serve as a preceptor for pharmacy students, residents, and healthcare learners, and mentor, orient, train, and assist with performance evaluations of pharmacy teammates, residents, and students. They will direct and check the accuracy of the work of pharmacy support team members to ensure safe and efficient workflows, service standards, and regulatory compliance. The pharmacist must demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served, including understanding the principles of growth and development over the life span and assessing data reflective of patient status to identify age-specific needs.

Requirements

  • Licensed in the state(s) of practice or eligible for reciprocity required.
  • Immunization and non-vaccine injectable certification required based on area of practice.
  • Credentialing and privileging required based on area of practice.
  • Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school of pharmacy required, or an FPGEC certificate from NABP.
  • No experience required.
  • Maintains skills in area of practice.
  • Strong computer skills.
  • Excellent communication and organization skills.
  • Excellent presentation, facilitation, and training development skills.
  • Demonstrates decision making, team building, customer service, and conflict resolution skills.

  • At least one year of residency.
  • PGY-2 Internal Medicine preferred or equivalent experience.

Responsibilities
