About The Position

Under general supervision of a Pharmacy Supervisor, this role is responsible for the interpretation, evaluation, preparation, and dispensing of medications. The Clinical Pharmacist Inpatient collaborates with multidisciplinary care providers to ensure safe, appropriate, and cost-effective medication use. This position also oversees and reviews the work of pharmacy technicians and ancillary support personnel. Requirements

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy.
  • Current unrestricted licensure as a licensed pharmacist or a licensed 90-day temporary pharmacist in the State of Rhode Island (must obtain full licensure prior to expiration of temporary license).
  • Must possess and maintain current American Heart Association Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) Certifications within 120 days of hire.
  • Effective interpersonal and communication skills, both written and verbal.
  • Effective organizational and presentation skills.
  • Knowledge of area/specialty specific technology, including pharmacy information systems, automated dispensing technology, software, equipment, and automation.
  • Experience with MS Word, Excel, Outlook, and web browsers.

Nice To Haves

  • Successful completion of Post Graduate Year 1 (PGY-1) Residency from an accredited program and/or national Board Certification in Pharmacotherapy preferred.
  • Post Graduate Year 2 (PGY-2) Residency from an accredited program in area of specialty strongly preferred, if applicable.
  • Clinical experience, knowledge, and development may be considered in lieu of formal training.

Responsibilities

  • Receives patient medication orders as written by a physician or practitioner.
  • Reviews and interprets each order to determine completeness, therapeutic appropriateness, and potential interactions.
  • Contacts the prescriber as necessary to initiate appropriate interventions for therapeutic efficacy, risk minimization, and cost-effectiveness.
  • Applies pharmacotherapeutic and pharmacokinetic principles to ensure safe and effective medication use.
  • Provides drug information and consultation to healthcare professionals regarding drug therapy, interactions, side effects, and incompatibilities.
  • Proactively reviews and approves medication orders in LifeChart, ensuring all relevant patient parameters are considered.
  • Supervises work performed by pharmacy interns and technicians to ensure performance standards and regulatory compliance.
  • Identifies, documents, resolves, monitors, and prevents drug therapy related problems.
  • Engages with other clinical pharmacists for complex clinical situations.
  • Integrates patient-focused medication therapy management and pharmaceutical care.
  • Participates in prospective and retrospective pharmacokinetic dosing and monitoring.
  • Ensures compliance with target drug programs, formulary management, and adverse drug event monitoring.
  • Documents and reports clinical interventions, medication errors, near misses, and adverse drug events.
  • Participates in emergency code blue response.
  • Attends and participates in medical, multi-disciplinary, and specialty rounds.
  • Reviews and approves formulations for sterile products requiring compounding or manipulation.
  • Completes in-process and final checks for medications prepared by pharmacy technicians, ensuring accuracy, sterility, and stability.
  • Participates in the education and training of healthcare practitioners, including residents, students, interns, technicians, physicians, and nurses.
  • Serves as preceptor to pharmacy residents and students.
  • Participates in continuous quality improvement activities and total quality management initiatives.
  • Participates in coordination of transitions of care.
  • Maintains professional competence through staff development and skills enhancement programs.
