About The Position

Cleveland Clinic Health System is seeking a Clinical Pharmacist - Inpatient to support Mercy's 24/7 pharmacy operations. This role provides both operational and decentralized clinical pharmacy services. Responsibilities include medication order verification, prescription fill verification, renal dose monitoring, and supporting IV room operations and medication automation systems. The pharmacist also performs compounding activities and collaborates with the onsite infusion center and oncology team to support safe and effective medication therapy. This position is eligible for a sign-on bonus up to $10,000 and relocation reimbursement up to $5,000. Requirements

  • PharmD or Bachelor of Science degree in Pharmacy from an accredited college of pharmacy.
  • Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire.
  • Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days (for individuals required to administer immunizations).
  • Current Advanced Cardiovascular Life Support (ACLS) and/or Pediatric Advanced Life Support (PALS) certification through American Heart Association (AHA) or American Red Cross - For caregivers required to respond to adult or pediatric cardiopulmonary arrest, this is required within one year of hire.

Nice To Haves

  • Board certification.
  • Completion of a post-graduate year 1 residency (PGY1), fellowship or relevant post-graduate pharmacy experience preferred.
  • Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area.

Responsibilities

  • Provide safe, efficient, patient-centered, evidence-based medication services.
  • Verify medication orders and/or dispense medications accurately according to state and federal regulations.
  • Document services in a complete, accurate and timely manner.
  • Ensure all pharmacy operations comply with national accreditation standards.
  • Maintain professional interactions with patients to facilitate successful patient experiences.
  • Educate and train students, residents, other caregivers, and/or patients.
