About The Position

Join our Inpatient Pharmacy Team at Lutheran Hospital in Wheat Ridge, Colorado, and become part of a collaborative group of more than 60 pharmacists and pharmacy technicians dedicated to delivering exceptional patient care. Together, we partner closely with physicians, nurses, and other care team members to provide safe, timely, and effective medication therapy across our 226-bed, state-of-the-art community hospital, designated as both a Level II Trauma Center and Comprehensive Stroke Center. Beyond supporting inpatient services, our pharmacy team also provides care for the Lutheran Collier Hospice Inpatient Unit and the Cancer Centers of Colorado Infusion Center at Denver West, creating unique opportunities to contribute across diverse patient populations and settings throughout the continuum of care. Pharmacists rotate between centralized and decentralized practice areas, offering a dynamic blend of operational and clinical responsibilities. Our team supports patient care through unit-dose dispensing, sterile IV compounding, participation in interdisciplinary rounds, and management of advanced pharmacy technology, including Omnicell automated dispensing cabinets and Swisslog Carousel and BoxPicker automation systems. Lutheran Hospital is a community-based, acute-care hospital in Wheat Ridge, Colorado. Lutheran Hospital began in 1905 as the Evangelical Lutheran Sanitarium, a tent colony for tuberculosis patients. As medical advances diminished the demand for tuberculosis treatment, community leaders decided the center should meet a new need. Community-based Lutheran Hospital, a not-for-profit, general medical facility located on a 100-acre campus, opened its doors in 1961. Construction of the six-story replacement hospital began in 2021, culminating in a state-of-the-art facility designed to support exceptional patient care, innovation, and growth. The new Lutheran Hospital campus at Clear Crossing in Wheat Ridge opened on August 3, 2024, and represents the future of healthcare in Denver's western suburbs.

Requirements

  • Doctor of Pharmacy or Bachelor of Science in Pharmacy degree from an accredited institution (education verified).
  • License to practice pharmacy in the state of practice and dispense controlled substances
  • Current knowledge of state and federal regulations and statutes regarding the practice of pharmacy in the state of practice.
  • Effective communication, people management, and project management skills.
  • Immunization certification if applicable to practice site needs

Responsibilities

  • Reviews patients' medication order for allergy interactions, duplication of therapy, appropriateness of drug therapy, and drug- drug interaction.
  • Makes appropriate evidence-based interventions to optimize medication use.
  • Ensures safe, appropriate, cost-effective therapies and accurately dispenses medications.
  • Oversees and/or participates in assuring appropriate medications are prepared and delivered timely to care areas and are stored both securely and appropriate for both stability and effective workflow.
  • Provides counseling to patients and promotes quality medication education and adherence.
  • Ensures patient is well informed, treated with respect, and receives the highest level of professional service.
  • Completes all regulatory and compliance training and ensures documentation of regulatory requirements is clear and complete.
  • Is knowledgeable and compliant with state and federal laws, regulations, pharmacy policies, procedures, and standard operating procedures.
  • Supervises and directs activities of interns, technicians, and support personnel as required.
  • Provides medication related clinical consultation and clarification to other health professionals.
  • Takes the initiative to solve problems including, but not limited to, equipment, maintenance, drug availability, staffing issues, and extraordinary patient care concerns.
  • Identifies potential areas for improvement including processes that could be streamlined or revised to improve patient satisfaction.
  • Identifies potential safety hazards that may create problems for patients and/or staff and takes appropriate actions.
  • Participates in departmental meetings, contributing ideas or feedback
  • Supports clinic or hospital and system directives, including clinical, compliance, business operations, standardization, and customer service.
  • Supports local and enterprise pharmacy leadership initiatives.
