About The Position

The University of Iowa Healthcare Urbandale Pharmacy is looking for a Clinical Pharmacist to join their team. This position is 90%25 time. The Clinical Pharmacist at the Urbandale Pharmacy provides pharmaceutical care to patients seen in UI Healthcare Ambulatory Care Pharmacies. The pharmacist provides clinical support, education, dispensing and monitoring of patients receiving medications through UI Health Care Ambulatory Care Pharmacies, including REI/IVF and Urbandale infusion services. Training to REI/IVF, Infusion, and Ambulatory Care Services is provided. The Ambulatory Care Clinical Pharmacist will serve as a role model for other Department of Pharmaceutical Care staff, residents, and students who look to improve their own professional capabilities and level of services, particularly in the provision and expansion of ambulatory pharmacy services. This role will be located at the Urbandale Pharmacy on 86th St and may include some additional centralized pharmacy support provided from the Urbandale Pharmacy location. Selective candidates are eligible for a sign-on bonus up to $10,000.

Requirements

  • One (1) year of applicable pharmacy experience is required. Prior work or student rotational experience may be considered as relevant.
  • Current licensure by the Iowa Board of Pharmacy Examiners required upon hire.
  • Excellent written and verbal communication skills.
  • Professional experience working effectively with individuals from a variety of backgrounds and perspectives.

Nice To Haves

  • Retail pharmacy experience.
  • Completion of an ASHP accredited PGY1 Residency.
  • Health system pharmacy experience.
  • Clinic order experience.
  • Board certification in Ambulatory Care, Pharmacotherapy, or other pertinent area.
  • Job-related experience with sterile compounding, USP 797, and USP 800 is desirable.
  • Immunization certification is desirable.
  • Experience providing customer service in a professional manner.

Responsibilities

  • Reviewing medication orders, monitoring therapy, and evaluating treatment plans within the electronic medical record to ensure appropriateness.
  • Providing patient education on medication regimens via in-person communication or virtual platforms.
  • Preparing, checking and dispensing medications, including sterile compounds and hazardous drugs when performing duties on-site.
  • Utilizing IV workflow technology to inspect and verify compounded sterile products.
  • Working with a multidisciplinary team to address patient needs and prevent adverse drug events.
  • Documenting all patient care activities and interventions in the electronic medical record which can be done through virtual means or on-site.
  • Adhering to all regulatory requirements, including applicable state and federal regulations, and accreditation standards, ensuring compliance at all times.
  • Facilitates Operational Initiatives Which Lead to Enhanced Patient Care Services
  • Addressing medication system issues and ensuring medication safety which may require remote review, data analysis and collaboration with healthcare teams.
  • Participating in hospital-wide quality programs and audits.
  • Supporting medication-related problem-solving and prescriber education through in-person and/or virtual consultations.
  • Contributing to the development of protocols related to appropriate prescribing, dispensing, and monitoring of medications administered in the infusion suite or therapy that is prescribed to the patient for home use.
  • Supporting sterile compounding operations by providing functional supervision of staff that work in and access the compounding area(s), primarily on-site with some incorporation of remote oversight where applicable.
  • Documents All Patient Care Activities Performed
  • Follows-up on any medication system problems, such as: Unclear or inappropriate prescription orders, Cost effective drug therapy, Prescription for non-formulary products
  • Carries out, as appropriate, Medication Use Evaluation Program initiatives through active prescriber intervention and education.
  • Serves on intra- and interdepartmental committees, task forces, and teams as required.
  • Facilitates completion of monthly clinic audits as assigned by the supervisor.
  • Assists with guidance of daily workflow.
  • Provides Educational Services
  • Serving as preceptor for pharmacy students and pharmacy residents.
  • Presenting in-service educational programs to staff nurses, physicians, and other colleagues on the healthcare team.
  • Presenting at and/or participating in other educational programs sponsored by the Department of Pharmaceutical Care, the College of Pharmacy, other UI Healthcare or UI departments, and state and national organizations.
  • Conducting educational classes for pharmacy students and pharmacy residents.
  • Conducting patient education classes about the proper use of medications.
  • Serving as mentor and role model for other pharmacist practitioners within the department, particularly those who provide ambulatory care services.
  • Participates in Research
  • Complete all required competencies and educational requirements.
  • Performs other duties as assigned, applicable to work setting and desired workflow.
