About The Position

Corewell Health is establishing a new infusion therapy location in Norton Shores to serve the greater Muskegon community. This new 7-chair clinic will provide non-oncology infusion therapies locally, expanding Corewell Health's service capacity. As a new location, this role offers the chance to be instrumental in building the clinic's culture, workflows, and patient experience from the ground up. Initially, the clinic will operate Monday through Friday, 8:30 a.m. to 5:00 p.m., with potential for future expansion into evenings and weekends based on patient volume and community needs. This clinic represents a significant new connection point to the Corewell Health system for many patients in the Muskegon area, offering a unique opportunity to establish a welcoming, high-quality, patient-centered experience from the outset.

Requirements

• Bachelor's Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy
• 2 years of relevant infusion/hospital pharmacy experience preferred
• American Society of Health-System Pharmacists (ASHP) accredited residency in Pharmacy Practice Preferred
• LIC-Clinical Pharmacist - STATE_MI State of Michigan 90 Days required
• LIC-Pharmacist Controlled Substance - STATE_MI State of Michigan 90 Days required
• CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association 180 Days required Or CRT-Pediatric Adv Life Support (PALS) - AHA American Heart Association 180 Days required Or CRT-Basic Life Support (BLS) - AHA American Heart Association 180 Days required Or CRT-Basic Life Support (BLS) - ARC American Red Cross 180 Days required
• PGY1 Residency or PGY2 Residency in Ambulatory Pharmacy preferred

Responsibilities

• Maintains accountability for ensuring the safety and efficacy of medication use.
• Educates and acts as a consultant to patients/family and members of the health care team related to optimal drug therapy.
• Participates in process improvement activities and drug use evaluations.
• Works with multiple medication automation systems.
• Interprets and screens medication orders for appropriate agent, dose, route, allergies, and potential drug interactions.
• Safely compounds and dispenses medications for administration to patients.
• Evaluates patients, orders appropriate laboratory tests, and interprets serum drug levels utilizing pharmacokinetic principles.
• Provides prescribers with dosage regimen recommendations to optimize therapeutic outcomes, reduce patient risk, and monitor the costs associated with drug therapy.
• Provides appropriate supervision and direction within appropriate skill level to technicians, residents, interns, and other support personnel.
• Engages in continuous study of the entire professional field to maintain the professional competence, knowledge, and skills necessary for satisfactory performance of all assigned responsibilities.
• Completes required continuous training and education, including department and Corewell Health specific requirements on a timely basis.
• Provides education and counseling to patients/family/significant others/members of the health care team in order to optimize outcomes and appropriate use of medications.
• Ensures that services are provided in accordance with State and Federal regulations, organizational policy, and accreditation/compliance requirements.
