About The Position

Join the pharmacy team at Beth Israel Deaconess Medical Center, a leading academic medical center and Harvard Medical School teaching affiliate. We are seeking a motivated Clinical Pharmacist to join our 7 on/7 off overnight team. This role focuses on ensuring safe, effective medication use through order verification, clinical review, and collaboration with a multidisciplinary care team. Pharmacists practice at the top of their license while supporting patient safety and operational excellence across all care areas. This position offers a highly desirable 7 on/7 off schedule, with potential flexibility to rotate into day or evening shifts based on departmental needs.

Requirements

PGY-1 Residency OR 1-3 years relevant hospital experience.

Responsibilities

Initiates and ensures the quality of pharmacy and drug distribution services in all patient care areas.
Assumes direct responsibility of direct medication distribution, IV admixtures, safety and efficacy of prescribed drug therapy in accordance with laws and regulations governing pharmacists and the practice of pharmacy by the appropriate state and federal agencies.
Practices in accordance with the state and federal laws, rules, and regulations, including but not limited to Massachusetts Board of Registration in Pharmacy, Department of Public Health, the Food and Drug Administration, and the Drug Enforcement Administration.
Maintains certification and credentials in accordance with the Board of Registration and adheres to institutional policies and procedures.
Reviews and verifies medication orders with consideration, including but not limited to the following patient specific parameters: diagnosis, weight/body surface area, age, allergies, organ function, laboratory values, medication duplication, drug interactions.
Clarifies orders deemed to be inappropriate, incomplete, or otherwise unclear with the prescriber in a timely manner.
Documents and communicates pertinent information and discussion with the healthcare team and pharmacy staff to ensure continuity of care.
Provides drug information to physicians, nurses and other health care professionals.
Checks and verifies sterile and non-sterile medications.
Supervises the activities of students, technicians, and residents within the pharmacist area of practice.
Promotes a culture of patient safety by submitting medication related events and near misses.
Completes daily pharmacy surveillance tool monitoring, including follow-up and documentation, as well as other daily patient care initiatives and relevant reports.
Works collaboratively with all pharmacists, pharmacy technicians, and pharmacy personnel.
Upholds a culture of respect and teamwork across the Pharmacy Department.
Participates in the onboarding and training of new hires and precepts pharmacy students and/or residents through core clinical rotations or learning experiences.
Regularly participates in department initiatives including but not limited to supporting formulary stewardship, presenting new medication requests at Pharmacy & Therapeutics Committee, creating and maintaining policies and procedures, conducting assessments of workflows, and/or developing departmental tools and resources.
Supports departmental workgroups to improve process and quality outcomes.
Supports the development and revision of departmental clinical guidelines.
