About The Position

The Clinical Pharmacist I initiates and ensures the quality of pharmacy drug distribution services in all patient care areas. This role assumes direct responsibility for direct medication distribution, IV admixtures, and the safety and efficacy of prescribed drug therapy in accordance with state and federal laws and regulations. Responsibilities include practicing according to regulatory bodies, reviewing and verifying medication orders based on patient-specific parameters, clarifying orders with prescribers, documenting and communicating pertinent information, providing drug information to healthcare professionals, checking sterile and non-sterile medications, supervising students, technicians, and residents, promoting patient safety through event reporting, completing daily pharmacy surveillance, and collaborating with the pharmacy team. The position also involves participating in the onboarding and training of new hires and providing mentorship.

Responsibilities

  • Practices in accordance with the state and federal laws, rules, and regulations, including but not limited to Massachusetts Board of Registration in Pharmacy, Department of Public Health, the Food and Drug Administration, and the Drug Enforcement Administration.
  • Maintains certification and credentials in accordance with the Board of Registration and adheres to institutional policies and procedures.
  • Reviews and verifies medication orders with consideration, including but not limited to the following patient specific parameters: diagnosis, weight/body surface area, age, allergies, organ function, laboratory values, medication duplication, drug interactions.
  • Clarifies orders deemed to be inappropriate, incomplete, or otherwise unclear with the prescriber in a timely manner.
  • Documents and communicates pertinent information and discussion with the healthcare team and pharmacy staff to ensure continuity of care.
  • Provides drug information to physicians, nurses and other health care professionals.
  • Checks and verifies sterile and non-sterile medications.
  • Supervises the activities of students, technicians, and residents within the pharmacist area of practice.
  • Promotes a culture of patient safety by submitting medication related events and near misses.
  • Completes daily pharmacy surveillance tool monitoring, including follow-up and documentation, as well as other daily patient care initiatives and relevant reports.
  • Works collaboratively with all pharmacists, pharmacy technicians, and pharmacy personnel.
  • Upholds a culture of respect and teamwork across the Pharmacy Department.
  • Participates in the onboarding and training of new hires and provides mentorship to students and residents.
