The Clinical Pharmacist I is a licensed healthcare professional dedicated to delivering safe and comprehensive patient care. In the Transitions of Care role, this individual oversees all aspects of the program, including reviewing pre-admission medication histories, counseling patients on discharge medications, and conducting post-discharge follow-ups. The primary goal is to enhance communication regarding medications, improve patient satisfaction, and minimize medication-related adverse events during care transitions. This role also includes overseeing pharmacy technicians, ensuring compliance with established processes, productivity goals, and key performance metrics. Additionally, the Clinical Pharmacist I provides training and education for pharmacy staff. Collaboration with colleagues, exceptional customer service, and serving as the primary medication expert for providers, staff, and patients are essential aspects of the position. Additional responsibilities may involve quality improvement initiatives or other duties as assigned by Pharmacy Leadership.

Requirements

Graduate of ACPE-accredited Pharmacy School
Current CA Pharmacist License
American Heart Association BLS certification

Responsibilities

Collaborate with providers, nurses, pharmacists, and other clinical staff to ensure the medication management process is safe, high quality, and cost-effective for all patients.
Verify pharmacy technician’s order entry of prior to admission medications into Epic.
Compare prior to admission medications with inpatient medications for discrepancies and contact the provider to resolve discrepancies.
Help identify medications that may need prior auth or special approvals for discharge and support the coordination around these medications.
Perform clinical review of discharge medications for appropriateness and counsel the patient or caregiver upon discharge.
Perform post discharge calls to patients within 48-72 hours to follow up on adherence and medication understanding.
Engages in activities that promote cost effective high-quality care.
Provides drug information and education to health care providers as needed.
Provides medication education and counseling to patients for all new medications as appropriate upon discharge and post discharge.
Actively participates in the education and development of pharmacists, pharmacy residents and pharmacy students as well as provides in-service education to nurses, physicians, and other healthcare providers.
Performs other related duties as assigned or requested.
