The Clinical Pharmacist I is a licensed healthcare professional dedicated to delivering safe and comprehensive patient care. In the Transitions of Care role, this individual oversees all aspects of the program, including reviewing pre-admission medication histories, counseling patients on discharge medications, and conducting post-discharge follow-ups. The primary goal is to enhance communication regarding medications, improve patient satisfaction, and minimize medication-related adverse events during care transitions. This role also includes overseeing pharmacy technicians, ensuring compliance with established processes, productivity goals, and key performance metrics. Additionally, the Clinical Pharmacist I provides training and education for pharmacy staff. Collaboration with colleagues, exceptional customer service, and serving as the primary medication expert for providers, staff, and patients are essential aspects of the position. Additional responsibilities may involve quality improvement initiatives or other duties as assigned by Pharmacy Leadership.
