About The Position

The Clinical Pharmacist - Transitions of Care is responsible for providing comprehensive medication management services (primarily to patients at high risk for readmission) through clinical services, patient and provider education, development of policies and procedures (as needed), and facilitation of drug distribution. Individual will be responsible for providing recommendations and/or assisting with ensuring safe medication use on patients' transition of care (i.e., discharge) from the hospital.

Requirements

  • Bachelors Pharmacy or Doctorate Pharmacy-Preferred
  • RPH - Reg Pharmacist
  • Pharmacist registration in state of Georgia within 180 Days Required
  • Minimum 3 years clinical pharmacy experience or PGY1 Pharmacy Residency completion Required
  • Strong verbal and written communication skills High
  • Strong computer skills. Experience with Epic, Microsoft Office, preferred High
  • Capable of preparing and providing presentations High
  • Strong organizational skills High

Nice To Haves

  • Board Certification (e.g., BCPS, BCACP, etc.) strongly preferred
  • Medication Therapy Management (MTM) certification Preferred
  • Experience with ambulatory care or retail pharmacy Preferred

Responsibilities

  • Provide recommendations on discharge medication reconciliation and/or perform Medication Therapy Management, in conjunction with the provider.
  • Document clinical interventions on an ongoing basis.
  • Demonstrate a basic understanding of distributive responsibilities, including computer functions.
  • Clarify medications upon admission and discharge, as needed.
  • Optimize medication selection to improve medication access post-discharge.
  • Counsel patients on their discharge medications/perform MTM services.
  • Develop and provide educational programs/inservices for health care practitioners and patients as required.
  • Provide drug information when needed or requested.
  • Serve as a preceptor/instructor for students from accredited colleges/schools of pharmacy.
  • Serve as a preceptor for the Pharmacy Residency Program (where applicable).
  • Participate on assigned committees and advance hospital initiatives, as needed.
  • Participate as a member of the Wellstar Pharmacy and Therapeutics (P&T) Committee, as well as other System and facility-specific committees as assigned or as deemed necessary.
  • Assist in preparing for and maintaining compliance with regulatory agency (e.g., The Joint Commission) standards/requirements.
  • Coordinate the dispensing of discharge medications, as needed.
  • Monitor metrics related to transitions of care services and develop improvement strategies as necessary
  • Other duties, as assigned.
