About The Position

We are seeking a proactive and collaborative Transitions of Care (TOC) Pharmacist to join our team. Reporting directly to the Retail Pharmacy, this specialized role is critical in bridging the gap between inpatient care and outpatient pharmacy services. The TOC Pharmacist is responsible for identifying and troubleshooting medication-related problems and potential barriers prior to discharge, providing comprehensive counseling, and promoting post-discharge support programs. Internally, you will advocate for UH Meds services through interdepartmental networking and collaboration with hospital teams. This is an ideal position for a clinician who excels at proactive problem-solving and cross-functional collaboration. A Brief Overview The Clinical Pharmacist is responsible for the safe, effective, and fiscally responsible use of medications through the application of clinical knowledge, skills, and professional judgment. The Clinical Pharmacist serves as a patient advocate when planning and implementing pharmacy care by considering each patient's disease states and background. These activities occur in all areas of pharmacy practice throughout the enterprise. The Clinical Pharmacist is legally responsible for the review and approval of all activities related to the dispensing of patient specific medications pursuant to a valid prescription or medication order from a practitioner as defined in ORC 4729, 4715, 4731, and/or 4741.

Requirements

  • Bachelor's Degree in Pharmacy (Required) or Pharm D degree. (Required)
  • Effective communication skills are essential (Required proficiency)
  • Computer and office equipment, automated compounding systems. (Required proficiency)
  • Registered Pharmacist (R.Ph) in the State of Ohio (Required)
  • Licensed practitioner of health sciences profession in the State of Ohio (Required)

Nice To Haves

  • Previous experience in the practice area with emphasis on computerized unit dose, sterile products, and patient focused clinical skills (Preferred)

  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
