About The Position

Responsible for the identification, resolution, and prevention of medication related issues. Provides drug information and recommendations for optimization of medication therapy to physicians, nurses and patients Develops competency in clinical practice including appropriate drug therapy for patients and prevention/resolution of drug related problems. Develops competency to evaluate patients for drug-drug, drug-food, drug-lab, and drug-disease interactions. Individualizes drug therapy regimens based on patient’s size, organ functions, and disease states. Facilitates medication-related continuity of care at admission, transfer and discharge. Reviews and accurately interprets medication orders. Develops competency in dispensing and compounding medications consistent with good pharmaceutical practice including compounding technique, packaging, and labeling. Practices in compliance with department policies & procedures, as well as local, state and federal regulations. Ensures formulary adherence, recommends cost effective therapies to providers, and minimizes waste in daily practice. Actively supports procedures and initiatives to prevent medication events. Reports events through appropriate channels. Actively supports department medication safety and quality improvement activities. Orients/trains new pharmacy team members. May work with interns, residents and students to guide daily activities. May be preceptor-in-training. Provides direction to pharmacy technicians to ensure smooth workflow and service standards.
