The Hematopoietic Transplant and Cellular Therapy Clinical Pharmacist will provide pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens. This role involves patient education, counseling on medication therapy, adverse effects, compliance, and administration. The pharmacist will design, recommend, monitor, and evaluate patient-specific pharmacotherapeutic regimens, implement recommendations, and document in the appropriate format. Responsibilities include reviewing and interpreting medication orders, participating in medical emergencies, supporting medication safety and quality improvement activities, and performing quality assurance. The role also involves collaborating with the healthcare team on formulary adherence and medication-use processes, assisting with the acquisition of medications, and practicing in compliance with regulations. Additionally, the pharmacist will prepare, dispense, and oversee medication storage and handling. This position also serves as a preceptor for pharmacy students, residents, and healthcare learners, and mentors, orients, trains, and assists with performance evaluations of pharmacy teammates. Directing and checking the accuracy of the pharmacy support team is also a key responsibility. The pharmacist must demonstrate knowledge and skills for age-appropriate patient care, understanding growth and development principles and assessing patient needs based on age-specific requirements.
