This Ambulatory Care Fellowship Program offers a unique opportunity for pharmacists to transition from retail or hospital settings into a clinical role with a focus on chronic disease management and patient care. The program has both 2-year options for new graduates and a 1-year option for experienced pharmacists without prior fellowship or residency experience. Fellows will establish professional relationships within medical practices, manage a patient panel to drive clinical outcomes, and utilize remote patient monitoring (RPM) for data interpretation. They will collaborate on medication management with supervising providers, lead discussions for staff and residents, precept students, and develop individualized care plans. Axil Health, the employing organization, is a collective of pharmacists providing clinical services within medical facilities, emphasizing chronic disease management, care coordination, and adherence monitoring. The role also involves participation in research projects evaluating disease states and new treatments.
