About The Position

The Pharmacy Department at Corewell Health Butterworth Hospital provides comprehensive pharmacy services for inpatients, the Emergency Department, Surgery, and the Infusion Center. This role is responsible for ensuring the safety and efficacy of medication use, acting as a consultant to patients, families, and healthcare teams on optimal drug therapy, and participating in process improvement activities and drug use evaluations. The position involves working with multiple medication automation systems, interpreting and screening medication orders for appropriateness, safely compounding and dispensing medications, evaluating patients, ordering laboratory tests, interpreting serum drug levels using pharmacokinetic principles, and making dosage regimen recommendations. The Clinical Pharmacist also provides supervision and direction to technicians, residents, and interns, engages in continuous professional development, and ensures services comply with all relevant regulations and policies. Education and counseling are provided to patients, families, and healthcare team members to optimize outcomes and medication use.

Requirements

  • Bachelor's Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy
  • 2 years of relevant experience hospital pharmacy

Responsibilities

  • Maintains accountability for ensuring the safety and efficacy of medication use.
  • Educates and acts as a consultant to patients/family and members of the health care team related to optimal drug therapy.
  • Participates in process improvement activities and drug use evaluations.
  • Works with multiple medication automation systems.
  • Interprets and screens medication orders for appropriate agent, dose, route, allergies, and potential drug interactions.
  • Safely compounds and dispenses medications for administration to patients.
  • Evaluates patients, orders appropriate laboratory tests, and interprets serum drug levels utilizing pharmacokinetic principles.
  • Provides prescribers with dosage regimen recommendations to optimize therapeutic outcomes, reduce patient risk, and monitor the costs associated with drug therapy.
  • Provides appropriate supervision and direction within appropriate skill level to technicians, residents, interns, and other support personnel.
  • Engages in continuous study of the entire professional field to maintain the professional competence, knowledge, and skills necessary for satisfactory performance of all assigned responsibilities.
  • Completes required continuous training and education, including department and Corewell Health specific requirements on a timely basis.
  • Provides education and counseling to patients/family/significant others/members of the health care team in order to optimize outcomes and appropriate use of medications.
  • Ensures that services are provided in accordance with State and Federal regulations, organizational policy, and accreditation/compliance requirements.
