About The Position

Community Medical Centers has been providing central California with the highest level of care available in the region. Community Regional Medical Center measures up with top hospitals nationally and is the 3rd largest in California! With 685 beds, we are home to the only comprehensive Burn and Level I Trauma center between Los Angeles and Sacramento. The 56,000 square-foot emergency department is one of the largest and busiest in California! We have an 84-bed Level III NICU and plan to significantly expand pediatric specialty care. Our hospital serves 2.5 million people within 9 counties, averaging a daily census of 641 patients. W're affiliated with UCSF as a teaching facility; trained 3,000 Medical Doctor's to date plus continuous invests in research and training grants.

Requirements

  • Doctor of Pharmacy (PharmD) required
  • RPH - Registered Pharmacist with valid State of California Pharmacist License required

Nice To Haves

  • Hospital pharmacy experience preferred

Responsibilities

  • Provides foundational pharmacy services, including medication order verification, dispensing oversight, drug information support, and basic patient-oriented clinical pharmacy services.
  • Reviews the accuracy and appropriateness of medications prepared and dispensed by pharmacy personnel and serves as a drug information resource to physicians, nursing staff, and other hospital personnel.
  • Supports safe, accurate, and efficient medication use throughout the hospital.
  • Verify the accuracy and appropriateness of routine medication orders and prescriptions, including allergies, interactions, compatibility, dosing, and other safety considerations.
  • Verify the accuracy of medications compounded and/or dispensed from pharmacy services, including carts, trays, kits, and other medication distribution systems.
  • Accurately verify sterile and non-sterile compounds, as applicable to assigned work area.
  • Provide drug information and basic consultative support to physicians, nurses, and other disciplines.
  • Perform patient-oriented clinical pharmacy services, including medication-related assessments, dose calculations, and recommendations for therapy optimization within established guidelines and protocols.
  • Perform protocol-based interventions, such as IV-to-PO conversions, as applicable.
  • Direct pharmacy technicians and support staff in completing work according to current priorities and workflow demands.
  • Document pharmacist interventions and report medication errors and near misses in accordance with department standards.
  • Serve as a resource to new employees, pharmacy students, and residents as assigned.
  • Perform other job-related duties as assigned.
