Clinical Partner - 12-Hour - Full Time - Days - (Hours: 11:00 am - 11:30 pm)
About The Position
The Clinical Partner (CP), under the direction of a licensed care giver, and after education and demonstration of competency, performs a variety of direct patient care activities or tasks specific to the unit/needs of the patient population. The CP maintains competency skills including the ability to provide care to special patient populations and patients with diverse cultural backgrounds. The CP can identify the ability of the adolescent, adult and geriatric patient to follow directions/instructions. The CP can identify safety concerns in the age related population and institutes additional measures to provide a safe environment; and identify developmental milestones in the age related population.
Requirements
- High School Diploma/GED - required
- Certified Nurse Assistant (CNA) Certificate - required
- Basic Life Support (BLS) Certificate from AHS or ARC - required
- Resume (include all CNA experience with start and end dates and contact information)
- HS Diploma/GED or equivalent
- California CNA License
- Basic Life Support Certificate (BLS) by ARC AHA
Nice To Haves
- Assoc. Degree/College Diploma - preferred
- 1 year Acute/Subacute / Convalescent /Nursing homes - Medical Center/ Hospital experience preferred
Responsibilities
- Identifies physical, emotional and social needs of patients and reports findings to the RN.
- Participates in the implementation of patient care and contributes to the plan under the supervision of the RN.
- Provides for the safety and comfort of patients, families and significant others.
- Reduces the risk of health care acquired infection by complying with current CDC hand washing guidelines and CSMC isolation policy.
- Safely uses restraints interventions under the direction of the charge nurse or primary RN.
- Provides therapeutic communication, re-direction, reality orientation, effective motivation of patients to participate in activities, and adequate conduction of rounds and observations to assure safety.
- Takes responsibility for reporting problems and/or patient complaints.
- Provides patient safety and comfort by utilizing resources and materials to safely transport patients using age-appropriate methods and equipment, uses two patient identifiers when transporting patient between units/ departments and when collecting specimen per scope of role, answer patients' call lights and requests, and report observations of the patient to charge nurse or primary RN.
- Maintains patient stability by checking vital signs and weight, recording intake and output, dispensing/retrieving patient care supplies, report abnormal vital signs to charge nurse or primary RN, as well as accurately documenting in electronic medical records.
- Ensures environmental cleanliness in patient rooms and patient care areas.
- Accompanies patients for any tests/procedures.
- Assists patients with with Activities of Daily Living (ADLs).
- Assist patient mobilization and out of bed activities.
- Identifies safety concerns in the age-related population and notifies licensed personnel of findings.
- Participates in all initiatives that assist in reducing the risk of patient harm by following safety precautions.
