Requirements

PharmD
Active Washington State pharmacist license

Nice To Haves

PGY1 and PGY2 (Oncology), or equivalent ambulatory oncology or infusion experience.
Board certification in an appropriate specialty (for example, BCOP).

Responsibilities

Review injectable and oral regimens for appropriateness, safety, interactions, and cost-conscious care, and provide consultative recommendations to prescribers.
Monitor therapy through patient assessment and lab interpretation, and help prevent and manage adverse effects and urgent medication-related issues.
Educate patients, caregivers, and staff on medication use, expected side effects, and monitoring requirements.
Support protocol development, guideline maintenance, and order set optimization in collaboration with the interdisciplinary team.
Support investigational drug and research-related medication processes as assigned.
Verify and dispense time-sensitive infusion medications and support safe preparation workflows consistent with sterile and hazardous drug standards, when applicable.
Help manage medication shortages and support infusion operations to maintain continuity of care.
