About The Position
Elite Care Management is seeking an experienced Home Health Clinical Manager (RN) to support our Indiana office. This role includes both office-based clinical leadership and field responsibilities to ensure high-quality, compliant patient care. Elite Care Management is a privately owned home health care company with over 26 years of experience in providing compassionate and personalized care to patients in their homes.
Requirements
- Active Indiana RN license
Nice To Haves
- 2+ years of home health experience preferred
- Clinical leadership or supervisory experience preferred
- Knowledge of OASIS and Medicare guidelines is a plus
Responsibilities
- Provide clinical oversight and case management for home health patients
- Review OASIS, plans of care, and clinical documentation
- Supervise and support field clinicians (RNs, LPNs, therapists, CNAs/HHAs)
- Perform supervisory and patient field visits as needed with rotating on-call
- Ensure compliance with Medicare, Indiana State Regulations, and accreditation standards
- Participate in QAPI, infection control, and quality improvement activities
