The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. This role involves delivering patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. The nurse will assess and evaluate patient care needs, apply critical thinking skills in patient care management, and integrate relevant assessment and intervention skills. Collaboration with the interdisciplinary healthcare team is essential for the development, implementation, and evaluation of the plan of care. The role requires prioritizing all aspects of patient care, including teaching, rounding, coaching, and planning after hospital care, and delegating to others as appropriate. Educating patients and caregivers about their plan of care, transition of care, health promotion, and disease prevention is a key responsibility. Effective and professional communication with patients, families, and the entire Interdisciplinary Patient Care Team is expected. The nurse will manage assignments within the Care Delivery Model of Modified Primary Nursing and demonstrate effective allocation of material resources.
