About The Position

Spectrum Healthcare Resources has an excellent opportunity for a civilian Full-time Clinical Nurse Specialist - Special Needs Coordinator at Vance Air Force Base in Enid, Oklahoma. This position emphasizes work-life balance in an outpatient environment. The role involves overseeing and managing clinical procedures for family members with special medical and educational needs, collaborating with multidisciplinary teams and service providers to ensure effective outreach and identification of special needs, finding appropriate medical resources, and providing optimal patient care. The coordinator will handle case assignments, review progress, determine case closure, help clients achieve wellness and autonomy, interact with clients, develop effective working relations with the medical team, and assist with continuity of care for specialty services and home health needs.

Requirements

  • Associate or Bachelor’s degree of Nursing.
  • Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), the Commission on Collegiate Nursing Education (CCNE).
  • ADN: At least 3 years experience in nursing and care coordination.
  • BSN: At least 2 years experience in nursing and care coordination.
  • Maintain an active, valid, current, and unrestricted license (with no limitations, stipulations or pending adverse actions) to practice nursing as a registered nurse in any US state/jurisdiction.
  • Current Basic Life Support Certification.

Responsibilities

  • Oversees and manages clinical procedures for family members that have special medical and educational needs.
  • Collaborates with a multi-disciplinary group and other key service providers, such as installation youth and childcare facilities and office and enlisted groups to ensure effective outreach and identification of special needs.
  • Finds the most appropriate medical resources and providing optimal care for the patient.
  • Handles case assignments, reviews case progress, and determines case closure.
  • Helps clients achieve wellness and autonomy.
  • Interacts with clients and develops effective working relations with the medical team.
  • Helps clients with continuity of care for specialty services and home health needs.
