About The Position

Under the supervision of the Charge Nurse and with clinical guidance from the treatment team, the Clinical Nurse, RN provides direct nursing care and supports the coordination and management of complex healthcare needs for a diverse patient population. The Clinical Nurse serves as a teacher, planner, advocate, and navigator for patients while performing nursing assessments, coordinating care, supporting continuity of services, and contributing to improved patient outcomes. The position may also assist with the supervision and delegation of duties to clinical support staff, including Medical Assistants and LPNs, as permitted by the nursing scope of practice.

Requirements

  • Graduation from an accredited school of nursing with a minimum of an Associate Degree in Nursing (ADN).
  • Current, active District of Columbia Registered Nurse (RN) license.
  • Current CPR certification.
  • ADN: Minimum of two (2) years of clinical experience.
  • BSN: Minimum of one (1) year of clinical experience.
  • Clinical experience may include emergency care, mental and behavioral health, women’s health, pediatrics, infectious disease, community or public health, care coordination, discharge planning, triage, chronic disease management, or patient education.
  • Strong communication, conflict resolution, and interdisciplinary teamwork skills.
  • Ability to work independently and effectively in a team-oriented environment.
  • Excellent interpersonal and written communication skills.
  • Strong organizational skills with flexibility and an action-oriented approach.
  • Ability to effectively communicate and work with diverse populations.
  • Proficiency with Microsoft Office Suite and other business software.

Nice To Haves

  • Candidates with an ADN must obtain a Bachelor of Science in Nursing (BSN) within five (5) years of hire.
  • Additional experience in teaching, health promotion, outreach, monitoring and evaluation of health outcomes, and working with diverse populations is preferred.

Responsibilities

  • Perform routine nursing procedures, including intake, triage, injections, and emergency care.
  • Conduct nursing assessments and evaluate acute and chronic health conditions, including cardiovascular disease, diabetes, and asthma.
  • Identify social and environmental barriers that may affect patients’ health and ability to access care.
  • Develop and support effective treatment plans and connect patients with appropriate healthcare, social, and community resources.
  • Coordinate patient care and follow up to promote positive health outcomes and continuity of services.
  • Collaborate with providers, Medical Assistants, social services/care support, registration, health information staff, and other members of the care team.
  • Provide patient education related to chronic disease management, blood pressure follow-up, family planning, immunization compliance, and other healthcare needs.
  • Provide discharge instructions and assist with coordination of care following hospitalization.
  • Coordinate quality control checks for glucometers and other waived testing.
  • Support infection control compliance in collaboration with the Nurse Manager and Medical Assistant III.
  • Monitor patients’ use of health center services and identify barriers to receiving appropriate care.
  • Document patient care accurately, timely, and in accordance with organizational policies and standards.
  • Execute and monitor data collection and reporting requirements related to institutional and federal regulations.
  • Participate in professional development opportunities, staff meetings, and team activities.
  • Assist with the supervision and delegation of duties of clinical staff, as permitted by the nursing scope of practice.
  • Identify opportunities for clinical improvement and communicate recommendations and action plans to appropriate personnel.
  • Facilitate home visits when appropriate for patients who are unable to schedule appointments at the Health Center.
  • Perform other duties as assigned.
