About The Position

To provide direct and indirect nursing services to patients and families. This role involves clinical advancement, utilizing systematic planning, appropriate methods, and timely exchange of information with the multidisciplinary team and patient/family to coordinate care. The nurse will demonstrate the ability to focus on and manage multiple patient needs and aspects of care, actively communicating with the care team to plan patient care. They will utilize available resources to problem-solve and determine the most effective approach to action. The position requires clinical judgment and decision-making, using the nursing process and a critical thinking approach to provide patient care that incorporates the mind, body, and spirit; is individualized, goal-directed, and consistent with current standards of safety. The nurse will identify actual/potential problems for which the patient is at risk, continuously evaluate interventions and care, and adjust the plan as patient and family needs change. They will identify the need for referrals based on information obtained in initial and ongoing assessments and evaluation, collaborating with the patient, family, and other team members to develop a comprehensive plan of care. Prioritization of demand for resources and collaboration with others to meet individualized patient needs is essential. The role may involve acting as a preceptor/teacher in guiding other nurses on how to apply the nursing process with a particular unit population. The nurse will implement safe, therapeutic, and efficient care for patients with complex needs due to multi-system disease and/or complications of treatment, demonstrating accountability for achieving patient outcomes. They will demonstrate an ability to quickly recognize a patient diagnosis even though presenting symptoms or situations may be dissimilar. Documentation will be per department/entity guidelines of care and policies and procedures, representing a detailed, concise picture of the patient's care. The nurse will serve as a resource for other staff in managing unit-specific documentation issues, with charting reflecting a critical thinking/problem-oriented approach to patient issues. Clinical leadership is also a component, involving delegation/assignment and communication of expectations for care delivery processes to each team member, with knowledge of what can and cannot be delegated. The nurse will be responsible for delegation and follow-through to ensure delegated tasks are complete, analyzing each situation before delegating and determining the level of supervision needed. They will assess the appropriateness of physician orders and question physicians when appropriate, helping coworkers improve performance, offering assistance and support, and providing positive feedback. The nurse will provide unit-specific information to students, floats, travelers/registry personnel and document appropriately, providing incidental teaching to team members. They will look for and apply ways to improve work processes and systems, offering and accepting constructive feedback/criticism in a non-judgmental, positive, and confidential manner, and seeking out opportunities to assist and support coworkers. Establishing a therapeutic relationship with the patient and family is key, anticipating problems and taking actions to enhance the quality of nurse/patient/family interaction. The nurse will analyze social/cultural variables and adapt nursing interventions to meet diverse needs, serving as a patient advocate. Providing age-appropriate patient education that facilitates recovery, self-care, end-of-life care, and health maintenance, promotion, and wellness is required. Coordination of transfer and discharge planning and teaching in collaboration with other members of the health care team is also part of the role. The nurse will collaborate with other team members to identify, develop, and/or revise patient education materials and demonstrate flexibility in teaching based on the assessment of learner's needs and the integration of adult learning principles.

Requirements

Graduate of nursing school
One year clinical experience (RNs with less than 12 months experience will complete the new grad requirements).
California Registered Nurse (RN) - CA Board of Registered Nursing
