The Clinical Nurse Manager is responsible for the clinical oversight of a caseload of clients. This role involves leading and supervising field staff to achieve specific outcomes for assigned cases. Key duties include ensuring compliance with clinical standards, policies, and procedures, performing case openings and assessments, medication reconciliation, creating clinical documentation, managing supply inventory, and updating medical status for patients. The position also requires maintaining compliance with accrediting agencies, state regulations, and funding source requirements.

Requirements

  • Bachelor’s degree required
  • Current Registered Nurse (RN) license in good standing.
  • Minimum 1+ year experience in nursing
  • Knowledge and/or experience in a home care model
  • Knowledge and/or experience with SCI, TBI, Respiratory failure, Pediatric Populations
  • Working knowledge of laptop/desktop PC
  • Proficiency in Microsoft Suite (Word, Excel)
  • Proficiency in G Suite (Gmail, Drive, Docs, Sheets, Google Meet)
  • Ability to follow written instructions
  • Ability to use computers and computer/software programs
  • Ability to communicate expressively and receptively
  • Demonstrated ability to work with an interdisciplinary team (Operations, Clinical, and Field Staff)
  • Superior customer service skills
  • Proven ability to effectively document findings of visit and communication with staff to meet objectives and paint a clear picture of client status
  • Highly organized with the capacity to work under tight deadlines and shifting priorities

Responsibilities

  • Perform Case Opens.
  • Monitor and proactively schedule clients for up to 15 in-home supervisory visits per week.
  • Prepare and process documentation related to performing visits.
  • Communicate with field staff to build relationships, offer support, and obtain feedback on client status.
  • Review shift notes to obtain recent history on client status and determine the quality of the record.
  • Work closely with the Director of Clinical Services and the Healthcare Team to provide support for staff and clients.
  • Provide a positive experience for staff and clients.
  • Perform Telehealth On-Call duties on a rotating basis.
  • Maintain client care plans including creating, sending, and tracking new/updated Plans of Care (POCs) and addendums.
  • Provide clinical education to the client, family/guardians, and field staff members.
  • Make administrative and procedural decisions and judgments on sensitive and confidential issues.
  • Document and report on patient progress, challenges, complaints, incidents, and status changes.
  • Manage inventory for supplies and medications on cases when necessary.
