About The Position

DaVita is hiring a Clinical Coordinator to lead outpatient dialysis care for patients with end-stage renal disease. The role oversees treatment, guides clinical staff, and ensures high standards of care and safety. The position involves coordinating patient care plans, monitoring outcomes, supervising clinical staff, including PCTs, and building long-term relationships with patients and families in a fast-paced, team-oriented environment.

Requirements

• Current RN license and CPR certification.
• At least 18 months of RN experience, including at least 6 months of dialysis experience.
• Charge RN readiness approval required.
• ADN required.
• Basic computer skills, including MS Word and Outlook.
• Flexible schedule, including weekends and holidays.

Nice To Haves

• BSN preferred.
• ICU, ER, or Med/Surg experience preferred.
• CNN/CDN certification is a plus.

Responsibilities

• Coordinate patient care plans and monitor outcomes.
