About The Position

The Clinical Nurse II (CNII) serves as the clinical consultant in patient care in a designated area and is assigned as a charge nurse. The CNII in consultation with the Nurse Coordinator plans, assigns and utilizes personnel effectively. Performs all the functions of the CNI positions and provides direct patient care to a population of patients. He/she identifies and implements nursing interventions and evaluates results for a variety of patient populations. Directs and acts as a resource to other members of the nursing team. Utilizes the nursing process in rendering care. Must be adept in performing all of the functions associated with the clinical unit, i.e. relief of telemetry monitor tech, equipment and clinical care. Must have documented/demonstrated clinical expertise in the caring for patients on the defined nursing unit, i.e., telemetry, emergency department, and medical surgical. Functions as resource person for new hires and less tenured staff. Promotes compliance and support of policies and procedures and achieves trust in professional collegial relationships. Skill in the use and operation of medical equipment used in providing specific care needs such as, cardiac monitors, telemetry monitors, pulse oximeter, ventilator. Recognizes standards for safety to reduce risk to individuals served.

Requirements

Licensed as a Registered Professional Nurse in the State of Illinois is required (Must provide proof at time of interview if already obtained)
Two (2) years of recent Emergency Department experience in an acute care setting is required
Current Cardiopulmonary Resuscitation (CPR) Certification is required (Must provide proof at time of interview if already obtained)
Current Advanced Cardiac Life Support (ACLS) Certification is required (Must provide proof at time of interview if already obtained)
Current Pediatric Advanced Life Support (PALS) Certification is required (Must provide proof at time of interview if already obtained)

Nice To Haves

Bachelor of Science in Nursing is preferred
Certification in assigned clinical area desirable is preferred

Responsibilities

Assesses and assists staff in assuring patients care needs are met.
Completely utilizes a patient classification system to assess the acuity level of each patient assigned.
Ensures that team members utilize the acuity classification system.
Assesses the patient care needs of their assigned patients.
Documents the biophysical, psycho-social, environment assessment on assigned patients that accurately portrays the patients’ problems.
Formulates nursing diagnosis on assessment data.
Promptly assesses/investigates patient care problems arising during assigned shift and follows the appropriate chain of command to resolve problems.
Assesses specific patient care assignments, with individual staff members to ensure quality patient care is maintained throughout the shift.
Participates in the development and implementation for the nursing care plan for each of their assigned patients.
Demonstrates competence in making nursing care assignments based on patient acuity levels and the skills and ability of the staff on the unit.
Collaborates with peers, supervisors and other health care team members in planning patient care.
Assumes a patient assignment.
Demonstrates skill in implementing nursing procedures required by the individual patient.
Performs the technical aspects of patient care according to established policies and procedures utilizing proper techniques.
Ensures that rounds are made at least once per hour to all patients on the unit.
Demonstrates an ability to respond and provide staff direction during crisis and emergency situation.
Directs emergency procedures well, ensuring that staff members are knowledgeable in emergency protocols.
Regularly checks emergency equipment or assigned to other personnel and ensures compliance.
Assists staff nurses and Patients Attendants as required.
Adapts work assignments around sudden changes in staffing.
Assists in coordinating patient admissions, transfers and discharges to provide for a safe, smooth operation and efficient flow pattern on the unit.
Ensures that the unit is a safe and clean environment.
Modifies nursing care plan as required with changes in patient conditions.
Evaluates effectiveness of nursing intervention in accomplishing goals and intended patient care outcomes.
Must maintain skills necessary for delivery of patient care and performs the technical aspects of patient care according to established policies and procedures, adapts techniques to the unique needs of individual patients.
Must maintain competencies in: EKG, and arrhythmic monitoring. IV therapy, infusion therapy.
Must assess patient and assign nursing staff based on skills needed for patient care in the patient care areas: OR/Recovery, Emergency Department, Telemetry, Medical Surgical.
Provides educational support for patients and their families.
Observes hospital dress codes and wears identification badge.
Provides proper notification of absences and tardiness.
Punctual for patient report and start of shift.
Maintains sound customer service standards.
Respects the rights, property and privacy of patients and others.
Recognizes the needs and behaviors of a variety of age groups. Integrates this knowledge and skills into age-appropriate responses to psychological, culture and physical needs.
Demonstrates nursing leadership skills and adaptability through performance of responsibilities.
Receives and gives appropriate and relevant Change of Shift report.
Consistently guides, directs and counsels staff regarding patient care, performance, problem solving, and decision-making.
Serves as resource to staff members in their daily activities.
Consistently functions as a patient advocate and clinical role model for other nursing personnel.
Demonstrates accountability for the proper storage of all controlled medications on the unit, as well as proper documentation of use and spillage.
Promptly investigates and resolves complaints of patients, visitors and other health disciplines. Makes referral to Nurse Manager/Tour Supervisor if unable to resolve.
Reviews and recommends changes in department and unit.
Responsible for effective communication and use of interpersonal skills.
Ensures continuity of care for all patients on the unit by appropriate verbal and written communication skills.
Regularly assists with patient teaching.
Handles telephone information and requests with courtesy, accuracy and respect for confidentiality.
Consistently develops and maintains an effective level of communication between and among staff.
Responsible for maintaining accurate documentation and medical records.
Demonstrates competency in the use of the appropriate forms and required reports.
Accurately utilizes and documents on the patient’s computerized medical record. Consistently documents and follows up on significant observations and problems with appropriate personnel.
Assists in identifying staff development needs of unit personnel.
Supports and participates in research programs approved by the Nursing Department.
Utilizes research findings to support and improve nursing practices.
Consistently advocates and functions within the guidelines of the Illinois Nurse Practice Act.
Maintains active membership in professional organizations appropriate to the area of nursing practice.
Maintains CPR certification and the ability to initiate, direct and perform life support measures in a Code Blue.
Demonstrates knowledge of current nursing practice and concepts relative to the area of practice.
Submits evidence of CEUs.
Recognizes own limitations and seeks help from appropriate sources as needed.
Conforms to department dress code appropriate to the area of practice.
