About The Position

Collaborates with Admissions, Pharmacy, Client Service Specialists, and other appropriate internal partners to support the safe, effective, and timely admission of patients to NuFactor services. Coordinates patient care activities and nursing schedules for an assigned caseload to promote continuity of care and timely service delivery. Serves as a clinical and operational resource to subcontracted nursing agencies, independent contract nurses, and internal staff.

Requirements

  • High school diploma or equivalent.
  • Completion of an accredited practical nursing or registered nursing program.
  • Working knowledge of nursing principles and practices applicable to patient care coordination and home-based services.
  • Working knowledge of clinical documentation standards and requirements for complete, accurate, and timely nursing documentation.
  • Knowledge of patient care coordination, continuity of care, and the clinical information needed to support treatment planning and reimbursement processes.
  • Working knowledge of applicable federal and state requirements governing nursing practice and patient care.
  • Knowledge of confidentiality requirements and the appropriate handling of protected health information.
  • Minimum of three (3) nursing experience.
  • Strong organizational skills with the ability to manage multiple priorities and maintain timely follow-through.
  • Strong attention to detail and demonstrated ability to prioritize work effectively.
  • Effective verbal and written communication skills.
  • Ability to work independently with limited supervision while collaborating effectively as part of a team.
  • Sound decision-making and consistently effective professional judgment.
  • Ability to exercise discretion and maintain confidentiality in accordance with HIPAA requirements and applicable privacy standards.
  • Intermediate to advanced proficiency in Microsoft Office applications.
  • Ability to identify, troubleshoot, and appropriately escalate clinical and equipment-related issues.
  • Full, current, and unrestricted license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the state in which the employee resides.

Nice To Haves

  • Knowledge of home infusion therapy, including commonly used medications, supplies, and ambulatory infusion pumps.
  • Knowledge of clinical care considerations for patients receiving immune globulin, hemophilia-related therapies, or other infusion therapies.
  • Familiarity with subcontracted nursing services, independent contract nursing, and coordination of external nursing resources.
  • Familiarity with home care operations and the clinical considerations involved in transitioning patients to the home setting.
  • Experience providing care to patients receiving hemophilia, immune deficiency, or other infusion therapies.
  • Experience in home infusion therapy, including familiarity with infusion medications, supplies, and ambulatory infusion pumps.
  • Ability to coordinate patient care activities across multiple internal and external stakeholders.
  • Ability to provide clear clinical guidance and direction to nursing resources and internal staff.
  • Ability to manage competing priorities in a fast-paced, service-oriented environment.
  • Ability to recognize clinical or operational issues requiring escalation and respond appropriately.
  • Ability to support effective communication with patients, families, physicians, nursing agencies, and other healthcare partners.
  • If licensed as a Registered Nurse (RN), must obtain Immune Globulin Certified Nurse (IgCN) certification within two (2) years of employment and maintain certification in accordance with applicable renewal requirements.

Responsibilities

  • Serve as a central clinical resource to internal and external staff regarding nursing and clinical matters, including providing telephone-based clinical support after hours and on weekends, as required.
  • Collaborate directly with physicians, clinical staff, patients, and families to obtain necessary medical documentation supporting treatment coordination, reimbursement justification, and continuity of care.
  • Facilitate the safe and effective transition of patients to home care services by proactively evaluating placement needs and the appropriateness of the patient and home environment.
  • Assist with the evaluation, coordination, and ongoing support of home infusion nursing services.
  • Support the development, coordination, and management of subcontracted nursing services and independent contract nursing resources.
  • Assist in the development, review, and revision of departmental policies and procedures, as needed.
  • Review clinical nursing documentation for appropriateness of therapy, patient outcomes, completeness, and adherence to established nursing documentation standards.
  • Demonstrate a proactive approach to assigned responsibilities, effectively managing multiple priorities and competing demands.
  • Provide appropriate direction and follow-up to help ensure assigned activities remain on track and are completed in a timely manner.
  • Participate in on-call schedules, special projects, committees, and other departmental initiatives, as required.
  • Maintain current knowledge of industry trends, clinical practices, and applicable therapies through review of relevant literature, completion of continuing education requirements, and participation in in-services, seminars, conferences, and other professional development opportunities.
  • Continuously strive to improve processes, service quality to internal and external customers, and employee relations.
