The Clinical Nurse Coordinator (CNC) is a front-line care provider, coordinator, and leader for the healthcare team. The CNC applies the nursing process at the point of care through strategic assessment, diagnosis, intervention, and evaluation of the unit to guide efforts for systematic, quality, and safety improvements. A key component of the CNC role is improving interdisciplinary communication and coordination at the point of care. As a care-delivery-systems expert, the CNC assists patients and families in navigating the healthcare system, advocating for both patients and families, as well as healthcare providers within the unit, hospital, and system. The CNC is accountable for improving care delivery within a unique healthcare setting, with a core focus on maximizing safe, quality, evidence-based care within complex systems.
