About The Position

The State of Connecticut, Department of Mental Health and Addiction Services (DMHAS) is hiring for a Clinical Nurse Coordinator (Psychiatric) to join their team at Connecticut Valley Hospital (CVH) in Middletown, CT. This full-time, 40-hour per week position is on the first shift, Monday through Fridays, 7:00 am - 3:30 pm. The role involves serving within the CVH Ambulatory Care Services Department, contributing to infection prevention, supporting pain and palliative care, providing wound care expertise, and performing data collection and analysis to improve patient care outcomes. DMHAS is a healthcare agency focused on improving the lives of individuals with behavioral health needs, and CVH is the largest DMHAS facility providing comprehensive inpatient and outpatient psychiatric care and substance abuse treatment.

Requirements

Possess and retain a license as a Registered Nurse in Connecticut OR hold a Multistate or Compact Registered Nurse license.
Three (3) years of professional nursing experience.
Considerable knowledge of psychiatric nursing and clinical therapeutic models of individual care with emphasis on interdisciplinary team approaches.
Knowledge of psychiatric illnesses and medications.
Knowledge of individual treatment planning including impact of medical insurance program requirements for reimbursement.
Knowledge of structure of services for individuals and their families in institutional and community-oriented settings.
Knowledge of aftercare facilities.
Considerable interpersonal skills.
Considerable oral and written communication skills.

Experience coordinating medical care for clients with mental health and addiction disorders.

Experience collaborating and developing treatment plans for clients with ACS medical providers.

Responsibilities

Serve within the CVH Ambulatory Care Services Department.
Contribute to the Infection Control Team through active participation in infection prevention functions.
Support pain and palliative care initiatives as part of the clinical service team.
Provide wound care expertise and clinical oversight.
Perform detailed data collection, analysis, and interpretation to support clinical and operational decision-making.
Develop, format, and present clear, accurate reports based on collected data.
Apply strong analytical skills to identify trends and help improve patient care outcomes.
Writes clinical program objectives, policies, procedures and manuals.
Conducts individual health assessments.
Develops, implements and reviews individual health care plans.
Participates or chairs individual-centered interdisciplinary teams.
Interviews individuals.
Accompanies individual on visits to treatment programs or placement settings.
Follows up on individual care in community programs to assure individual in least restrictive environment.
Assures continuity of care to prevent recidivism.
Applies for financial aid for individual through Department of Social Services.
Maintains records of individual progress or failure after placement.
Develops community health resources and provides on-going consultation and monitoring of services.
Acts as a resource person to staff and community.
Prepares reports and statistical summaries.
May clinically supervise direct care staff in execution of clinical programs.
May develop and conduct in-service educational programs for staff and families.
May perform recordkeeping functions.
Performs related duties as required.
