RN V - Clinical Nurse Coordinator MDS

About The Position

The Massachusetts Veterans Home (MVH) at Chelsea is actively seeking a Clinical Nurse Coordinator - MDS (Registered Nurse V) to join their team. This position performs the duties of a minimum data set (MDS) coordinator by assuming overall responsibility of the MDS process. You will evaluate nursing activities of all shifts by reviewing patient charts, observing nursing care, visiting patients to ensure that nursing care is carried out as directed, and treatment is administered in accordance with physician's instructions. In this role. you will also coordinate the implementation of veteran's' individual treatment plans, as determined by an interdisciplinary team, by conferring with appropriate health care professionals. In addition, you will coordinate the clinical activities of the facility. Please see below for more detailed list of duties & responsibilities.

Requirements

  • Experience in a nursing leadership position and/or credits towards a graduate degree in nursing.
  • Ability to keep current on changes in MDS and regulations affecting nursing practice.
  • Background in the application of evidence-based practice and research translation.
  • Experience with regulatory compliance, including CMS standards.
  • Expert knowledge of geriatric age specific care and geriatric nursing.
  • Strong clinical acumen regarding the physical assessment and pathophysiology regarding chronic as well as acute illness in the geriatric and Veteran population.
  • Efficient time management and decision-making skills.
  • Proficient to expert level use of technology, including EMR.
  • Ability to teach and mentor others.
  • Ability to manage conflict and collaborate with others across the multidisciplinary care team.
  • Advanced knowledge of the nursing process, care planning, and change management.
  • Leadership skills, including effective verbal and nonverbal professional communication, critical thinking, conflict management, and effective decision making.
  • Applicants must have at least (A) three years of full-time, or equivalent part-time, experience as a registered nurse in a recognized hospital, clinic or medical facility and (B) of which at least two years must have been in a supervisory, administrative, or managerial capacity, or (C) any equivalent combination of the required experience and the substitutions below.
  • Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing .
  • Recent graduates from an approved school of nursing must have applied for the first available registered nurse examination.
  • Based on assignment, possession of a current and valid Massachusetts Motor Vehicle Operator's license may be required.
  • As a condition of employment, depending on job assignment and location, employees may be required to have received COVID-19 vaccination and/or other vaccinations, or to have been approved for a vaccination exemption prior to their start date.

Nice To Haves

  • Three (3) to five (5) years experience with coding, billing, and documentation for PDPM.
  • Increased understanding of VA, MA, and CMS regulations regarding long-term care (LTC).
  • Increased understanding and knowledge of The Joint Commission (TJC) standards and accreditation process.
  • Long-term or rehabilitative care experience

Responsibilities

  • Responsible for collaborating and communicating with appropriate departments to ensure that all sections of the MDS and corresponding assessments are completed by reference date.
  • Educates staff on the MDS process, conducts audits, monitors compliance, collects data, and submits reports.
  • Coordinates the MDS schedule with respect to its planning and execution with the nursing team.
  • Functions as an MDS resource for all departments involved with the resident assessment instruments (RAI) process.
  • Educates new Registered Nurses (RNs) on the MDS process.
  • Completes PI audits on the MDS process, related assessments, and quarterly summaries by all shifts.
  • Assists with the scheduling and planning of IDC meetings to include notification of participants.
  • Oversees compliance regarding the completion and documentation of quarterly summaries and significant changes and all associated assessment tools, including but not limited to the Norton Scale, Risk Fall Assessments, Smoking and Wander Risk Assessments.
  • Assist the quality improvement team with data collection and help with developing corrective action plans.
  • Performs the duties of a Registered Nurse V when needed.
  • Assists with admissions/readmissions of veterans, when assigned.
