Clinical Navigator (RT)

Respire Homecare Services•Elmhurst, NY

About The Position

Respire Homecare Services is hiring a Clinical Navigator (RT) to guide patients and families through the home-care journey, from referral acceptance through onboarding, equipment setup, ongoing support, resupply, recertification, and discharge or transfer. The Navigator serves as a consistent clinical guide and point of contact, coordinating with families, Sales Liaisons, providers, referral partners, payers, and internal teams to anticipate barriers and keep each step moving. The role is assigned to 2–4 Sales Liaisons and their shared patient portfolio, and reports to the Director, Patient Navigation. This is hands-on clinical navigation rather than traditional intake. The Navigator uses respiratory therapy expertise to educate and reassure caregivers, troubleshoot respiratory equipment and therapy concerns, coordinate transitions home, and support field setups and follow-up visits when needed. The role also manages clinical and payer requirements, insurance verification and prior authorizations, documentation, continuity outreach, and escalation of concerns within role scope. Respire describes its culture as mission-driven and focused on patient-centered care, whole-person wellness, dignity, teamwork, trust, open communication, and mutual respect. The company offers opportunities for career growth and a competitive pay and benefits package. Compensation is $38–$45 per hour.
