The Oncology Clinical Navigation Specialist provides individualized assistance to patients, families, and caregivers throughout their cancer journey from diagnosis through survivorship. The Navigation Specialist works directly with Oncology providers, serving as a consistent care coordinator and patient advocate to ensure timely access and seamless transitions across the oncology care continuum. Responsibilities include facilitating rapid scheduling, coordinating care across service lines and sites of care, identifying and removing barriers to care, and tracking patients to ensure compliance with treatment and surveillance plans. The specialist provides patient and family education regarding disease processes and available support services and resources. They act as a clinical expert in facilitating and delegating all care aspects, participate in accreditations to ensure compliance with standards, and provide billable services in conjunction with the lead provider. Excellent communication and facilitation skills are required, and an ideal candidate is self-motivated with strong leadership and critical thinking skills.
