About The Position

The Clinical Manager HC/LTC is responsible for managing an assigned patient care service area composed of nurses, hospice aides, social workers, chaplains, volunteers, and administrative staff. This role provides direct supervision to RN Case Managers, Hospice Care Aides, and administrative personnel while ensuring quality, cost-effective care delivery. The manager oversees staffing, performance, compliance, and service quality, serving as a clinical and operational leader within the home care/long-term care setting.

Requirements

  • Strong knowledge of nursing principles and hospice care.
  • Effective communication (written and verbal).
  • Proficient in problem-solving, prioritizing tasks, and supervising teams.
  • Ability to maintain compliance and lead quality improvement efforts.
  • Skill in public speaking and interdisciplinary collaboration.
  • Minimum of three (3) years clinical nursing experience; two (2) years in hospice preferred.
  • At least one (1) year of supervisory experience required.
  • Active RN licensure in the State of Florida.
  • Valid Florida driver’s license with acceptable driving record.
  • Medical clearance for communicable diseases.
  • Current immunizations (e.g., TB, Hepatitis, Flu).
  • Ability to work long/irregular hours as needed.
  • Associate degree or Bachelor’s degree in Nursing from an accredited institution.

Nice To Haves

  • CHPN certification preferred.

Responsibilities

  • Coordinate, plan, implement, and evaluate interdisciplinary patient care.
  • Supervise and support staff through orientation, training, evaluations, and feedback.
  • Track quality indicators and lead performance improvement initiatives.
  • Monitor clinical treatment and progress, audit patient records, and escalate care concerns as needed.
  • Oversee patient assignments based on acuity and location; ensure adequate staffing.
  • Lead weekly IDG (Interdisciplinary Group) meetings and participate in community engagements.
  • Provide expertise in hospice care; educate staff and external stakeholders.
  • Maintain workspace quality and ensure compliance with standards and regulations.
  • Manage team scheduling, PTO approvals, and labor budgets.
  • Handle payroll submission and staffing updates.
  • Support service recovery efforts and address complaints/occurrences.
  • Maintain compliance with state and federal regulations.
  • Provide weekend/holiday coverage as needed.
  • Perform other related duties as assigned.
