About The Position

The Clinical Manager role is responsible for the day-to-day oversight of clinical operations as well as daily patient care. Excellent oral and written communication skills are required. Supervisory skills are a must. The Clinical Manager reports to the appropriate HCO Service Line Director.

Responsibilities

  • Supervise and manage the clinical operations of the site. Clinical operations include maintenance of: supplies, facility, regular hours, consistent staff, lab services, patient charts, follow-up logs, patient education and counseling services, patient education materials, staff continuing education, new employee training, budget, scheduling, etc.
  • Conduct self in a professional manner and propagate professionalism among other site staff.
  • Actively participate in the medical home care team which is comprised of clinical and non-clinical staff working together towards a common goal: delivering quality care to our patients.
  • Communicate and coordinate effectively with all members of the care team.
  • Provide self-management support in the form of patient coaching and motivational interviewing when appropriate.
  • Maintain compliance with the organization’s confidentiality policy in accordance to the Health Insurance Portability and Accountability Act (HIPAA).
  • Identify family and environmental patient problems, and work with local health agencies and professionals in their solution.
  • Apprise the county health department by standard reporting procedure of any patient problem having public health implications.
  • Assist in care coordination which may include obtaining test and referral results and communication with various entities including but not limited to community organizations, health plans, facilities and specialists.
  • Proactively address needs of the patients and families through the concept of population management.
  • Effectively communicate with all patients with specific attention to the vulnerable patient population.
  • Facilitate and maintain communication among staff and administration, i.e. regular staff meetings, participation in employee evaluations.
  • Responsible for services provided by nursing staff. Includes patient flow, time management, patient courtesy, quality of patient education, current licensure, etc.
  • Responsible for services provided by adjunct service staff. (Nutrition, security, housekeeping, etc.) Includes: time management, professionalism, patient courtesy, supplies, reliability, thoroughness of work, etc.
  • Provide general supervision of office staff and clinical staff, jointly with the office Manager.
  • Uphold Valley Health policies and procedures and personnel guidelines.
  • Communicate maintenance/capital needs for site to Operations team. For example, furniture, copier, lawn care etc.
  • Maintain an attractive, pleasant environment at the site.
  • Maintain community support through communication and networking. e.g. parent/site relations, community/site relations.
  • Include in time as appropriate for direct patient care and outreach program participation.
  • Delegate responsibilities as appropriate.
  • Participate in the interview/hiring and orientation process of future staff.
  • Establish and maintain an appropriate data collection system. Data needed include demographics, rates of occurrences, etc.
  • Convey a professional and positive image and attitude regarding the health center and organization.
  • Develop good rapport with staff.
  • Perform other duties as assigned.
