The Clinical Manager is responsible for the day-to-day triage of team activities, including accepting referrals, ensuring timely admissions, assigning patients to appropriate team members, and monitoring patient care as ordered on the interdisciplinary plan of care. This role involves conducting IDT meetings, supervising and supporting clinical staff through evaluations and field supervisory visits, and ensuring the ongoing development of the teams' clinical skills. The Clinical Manager will also ensure quality care and compliance with state and federal regulations, CHAP accreditation standards, and the organization's policies and procedures. This position supervises applicable Case Managers, RNs, LPNs, CNA/HHAs, Therapists, Social Workers, Grief and Bereavement Coordinators, and Chaplains.
