About The Position

This is a high-priority leadership position for a qualified Registered Nurse (RN) focused on coordinating, leading, and supporting patient care services within a structured home health setting. Operating in a hybrid capacity out of San Antonio, TX, you will take complete ownership of caseload planning, referral reviews, clinical order approvals, and clinical staff alignment while strictly adhering to compliance and regulatory standards.

Requirements

• Minimum of one (1) year of recent clinical experience as a Registered Nurse (RN).
• At least one (1) year of dedicated Home Health experience is strictly required.
• Thorough understanding of Home Health operations, Medicare Conditions of Participation (CoPs).
• Hands-on proficiency with Homecare Homebase (HCHB) software systems.
• Must possess a valid Driver's License for Screening and Selection Services (SASS) and complete a Candidate Attestation.

Responsibilities

• Coordinate, monitor, and oversee comprehensive care planning for all patients under your assigned caseload.
• Review all incoming new patient referrals for clinical appropriateness, and approve/co-sign clinical orders (including LVN and follow-up orders).
• Facilitate case conferences, maintain communication with physicians, schedulers, therapists, and field staff to ensure continuity of care.
• Ensure timely submission of assessments, align documentation with billing guidelines, and coordinate corrections directly with the QAPI team.
• Support the Director of Nursing by supervising field staff, assisting with scheduling, and participating in call rotation/triage responsibilities.
