About The Position

The Clinical Manager provides strategic leadership and oversight for clinical care management operations, ensuring the delivery of high-quality, person-centered services to older adults and individuals with complex healthcare and social service needs. This leadership role is responsible for guiding clinical supervisors and care management teams, supporting staff development, driving quality improvement initiatives, and strengthening interdisciplinary collaboration across programs and community partners. This position is ideal for a licensed Social Worker or Registered Nurse with strong experience in care management, clinical operations, and team leadership who is passionate about improving outcomes, mentoring staff, and advancing innovative service delivery within a mission-driven organization. Requirements

  • Current licensure as a Social Worker by the State of Ohio Counselor and Social Work board or licensed as a Registered Nurse by the Ohio Nursing Board with preference to LISW or to BSN or RN with MA in related field.
  • At least five years paid work experience as a medical social worker/counselor or nurse in a human service/health care organization, at least three positions working with older persons.
  • Minimum three years supervisory experience.
  • Knowledge of community resources, assessment, and care management and excellent writing and interpersonal communication skills are essential.
  • Basic knowledge of Microsoft Windows applications.
  • Must have a car available during working hours, as well as the ability to legally drive a car.

Responsibilities

  • Provide direct supervision of clinical supervisors, providing coaching and performance evaluation of supervisory staff and indirect supervision of Care Managers.
  • Assist clinical staff in interpretation of policies and procedures, as well as program implementation and problem identification as it relates to service delivery.
  • Responsible for maintenance of minutes of clinical staff meetings and the development of new/revised policies and procedures as may be required by clinical program changes.
  • Interface with provider operations staff to ensure necessary communication regarding utilization/quality assurance and clinical issues related to service delivery.
  • Initiates, facilitates, and participates in case conferences with other agencies to address and resolve clinical service issues.
  • May be asked to support practicum experiences of students from educational institutions.
  • Assist with other management staff as needed in the training of new staff.
  • Identify training needs.
  • Participate as part of the management team to address policies and procedures relative to service delivery.
  • May participate on community organization boards and committees dealing with aging and care management services to provide an opportunity for networking and community liaison opportunities.
  • Represent the Agency in speaking to community groups as requested.
  • Perform other related duties as assigned by the Program Director.
  • This position's primary duty is to perform work directly related to the successful operation of the Agency. Accordingly, the employee must exercise discretion and independent judgment with respect to matters of significance.
  • All staff have the responsibility and authority to propose ways to improve quality and to fully participate in quality improvement efforts.
