About The Position

Croí Health is a mission-driven, non-profit organization providing home health, palliative, hospice, and wellness services across 27 communities. We are seeking an experienced, talented, and compassionate Home Health Clinical Manager for a full-time position. The Full-Time Home Health Clinical Manager is responsible for the overall day-to-day operations of the Home Health Department, including oversight of clinical programs and staff. This role provides leadership, supervision, and support to an interdisciplinary team while ensuring the delivery of high-quality patient care in accordance with the Agency’s mission, values, policies, and regulatory requirements.

Requirements

  • Current Massachusetts PT or OT license.
  • Experience working with electronic medical records required.
  • Minimum of 2–3 years of PT or OT experience experience.
  • Home Health experience required.
  • Minimum of 1 year of supervisory experience, preferably leading a team of nurses and clinical staff.

Responsibilities

  • Supervise clinical programs and professional/clerical staff.
  • Ensure the delivery of high-quality, patient-centered home health services in compliance with state, federal, and agency regulations.
  • Communicate effectively and compassionately with patients, families, physicians, and community partners regarding care plans and services.
  • Collaborate with Intake and Referral teams to ensure appropriate home health admissions and timely initiation of care.
  • Maintain appropriate staffing levels; participate in hiring, onboarding, performance management, and termination processes.
  • Ensure staff competency through orientation, education, coaching, and performance evaluations.
  • Coordinate patient care conferences, case management activities, and interdisciplinary team meetings.
  • Provide timely coaching, counseling, and recognition to staff members.
  • Participate in home health marketing, community outreach, and referral development activities.
  • Assist with budget development, approve care-related expenditures, and implement cost-effective practices while maintaining quality patient outcomes.
  • Contribute to short- and long-term departmental planning and strategic initiatives.
  • Monitor patient satisfaction, conduct client visits as needed, and investigate and resolve patient or family concerns.
  • Submit required departmental reports within established timeframes.
  • Support clinical quality improvement initiatives, regulatory compliance, accreditation readiness, and survey preparation in collaboration with the Quality Improvement Department.
  • Monitor key performance indicators, including patient outcomes, productivity, utilization, and quality measures.
