About The Position

Clinical Liaison (RN or RT license required) at Select Specialty Hospital, a Critical Illness Recovery Hospital (LTACH) in Atlantic City, NJ. This full-time position involves advocating for referred patients in the pre-admission process by collaborating with clinical teams to assess patient needs. The role serves as a resource for healthcare professionals by fostering relationships within medical facilities. With clinical expertise and problem-solving skills, the liaison will also coordinate care plans with physicians and families. Additionally, the role facilitates patient transfers and addresses barriers to accessing specialized post-ICU care. The position is Monday-Friday, 8 am - 5 pm, with an on-call weekend rotation. At Select Specialty/Regency Hospitals, a division of Select Medical, we care for chronically and critically ill or post-ICU patients who require extended hospital care.

Requirements

  • Registered Nurse (RN) or Respiratory Therapist (RT) License.
  • Ability to complete clinical assessments of medically complex patients.

Nice To Haves

  • Will consider other designations like LPN, PT, OT, etc., upon further review.
  • Demonstrated success in patient advocacy.
  • Superior communication and relationship-building skills.
  • Prior healthcare marketing or business development experience is preferred, but extensive training is provided on this piece.

Responsibilities

  • Work with physicians, discharge planners, and patients' families to plan the best continuum of care after an ICU or post-surgical stay by utilizing strong clinical background, payer knowledge, and problem-solving skills.
  • Perform clinical assessments of referred patients, then counsel with patients' families to assist them in making the best care decisions for their loved one.
  • Partner with hospital case managers and physicians to create a smooth pre-admission and transfer process through removing payer-related and operational barriers for patients getting into our highly effective level of care.
  • Advocate for referred patients in the pre-admission process by collaborating with clinical teams to assess patient needs.
  • Serve as a resource for healthcare professionals through fostering relationships within medical facilities.
  • Coordinate care plans with physicians and families.
  • Facilitate patient transfers and address barriers to accessing specialized post-ICU care.
